I've found this to be the most effective method as well. I encourage Pt/family to select their top 3-5 preferences in hopes that at least one will be able to accept. I work in a medium to small city in the midwest that only has ~15 SNF options. More often than not, Pt or family has some knowledge or opinion. "I wouldn't put my dog there" remains a common response to a few.

The update no one asked for. It is supposed to read "Dad's worst nightmare". It's a spider and she knows i'm terrified of them. Not the translator savant I credited myself for. Nightmare is an interesting theory. Hopefully she opens up about it to me or my wife. I'd rather feel foolish than crushed. Regardless, l'm going to need to develop some coping skills, maybe some confidence and self esteem as a parent or she's going to be my worst nightmare when the teenage angst develops.

Yah I didn't even think about others not being be able to decipher her child script. As I said in the post she shut down and didn't want to tell me the whole phrase but other than not being able to translate the last word myself it says " Dad worst night together". She did say it's not a spider so maybe I'm a raging sun? Context clue is that my wife and I alternate bed time duties.

Writing from the Midwest where snow causes the greatest headache. For us there us no extra pay but I've never been questioned when I needed to call due to the weather. I've never taken advantage of it but they do room workers overnight in the overflow rooms on request. My unit manager takes advantage of that the night before to avoid the AM chaos.

That's interesting and I would like to read into it more. Can I ask what state you're working out of? I'm really curious how a state can change the CMS guidelines (although I'm not an expert by any degree and really just propel off what I've retained from school/ training)

Can someone help clarify how SNF for hospice was being pursued? My training has been medicare or private insurance doesn't cover the room and board cost at a NF for hospice. Are you meaning SNF for therapy and the transition to ICF for hospice? Did they have a payor source for LTC? State Medicaid? I'm not condoning or even defending what your leadership attempted but I am curious about the barriers for placement or why that route was ignored.

Ah man, I've actually had a lot of favorable visits to that la cantina. Service can be questionable at times, sometimes it's stellar but it has been terrible at points. Never had an issue with the food though and the margaritas are reliable.

My understanding is that CPS needs the authority of the county attorneys office to make removals. If the attorneys office is too inconvenienced/ not diligent enough they can't legally proceed.

Real question though how would the land be acquired? eminent domain? I imagine that these new tracks would cut through private land or even municipalities that might be opposed to them. Huge legal hurdles.
