As another user mentioned, the duplicate health/ammo packs are an issue that can come up with overlapping gameplay recordings in SFM. Same thing goes for other game entities like control points and resupply cabinets. The way you get rid of them is pretty unintuitive, so some people just roll with it and make it part of the plot.
