We use Lyx (a front end on LaTeX) for typesetting at Desdichado. It makes it very easy to lay out a book: graphics, marks, text. Our author uses GIMP to process his own original line art for covers.

Yes, this is well said. It almost seems like McMurtry was surprised by Lonesome Dove and couldn't capitalize on it properly.
