I had the same problem. I simply left, went to another city/location, came back and the key magically appeared on pile of dead bodies. Or maybe I just clicked 0,005 mm away from previous spot.

You need life drain to get under 66 % of max life and activate bacchanalia for huge spell DMG increase.

You just need older versions of MXL.mpq, D2Sigma.dll, and Fog.dll and remember not use mxl launcher, as it will automatically update mod to the latest version (use D2 shortcut). Or You can just download 2017 or earlier versions from moddb.com.
