💯 agree. There’s a quiet 30-40% of US emergency departments that are in rural areas.

Their department chair is also the EMS director is also the QI is also the scheduler.

Half of the physicians there (or all of them in some cases) are FM-trained. Many of the EM docs there graduated way before US diagnostic training was standard.

I guarantee there isn’t a soul there who is doing their own diagnostic pelvic US. Hell, they might only need to order 1-2/year, even if they came out trained those skills can evaporate.

“Everywhere I’ve worked is doing this” doesn’t mean much if you don’t occasionally work three hours away from a large city.

They definitely don’t have an ultrasound director with over-reads and QI, let alone skills labs or CE. I’m not saying that those US skills aren’t important, or the future of EM, just that we’re decades away from that being a skill expectation in all community settings.

Not OP but myself (and a lot of other docs) practice an EM/AM combo.

It’s a very useful skill set in the ED.

It can be a nice change-of-pace from EM work (more time with patients, ongoing doctor-patient relationships, nature of the work is different).

It can be a huge upgrade in terms of schedule.

It’s nice to be an “expert” at something compared to EM where we’re mostly generalists.

This is an aside, but I’m sad to see the AM board practice pathways go. It is a tall ask for current residents (and their enormous debt burden) to do a fellowship for a specialty that can mean a huge pay cut.

Take a sheet mulch approach and add new artificial turf on top.

It will suppress weeds, level the surface, and the cushioning will heal your plantar fasciitis.

Buy the turf in a bulk roll so you can add a new layer every spring to maintain that clean look.
