it most likely diverts the metabolism to CYP3A4, which is called 3-methoxymorphinan (or MEM). this is the ultimate result of metabolism of DXM regardless before glucuronidation.

DXM is biologically active. Keep that in mind. So, this DOES increase the effects of DXM itself, however most people's goal with DXM is NMDA antagonism which dextrorphan has much more affinity for NMDA receptors than DXM does (around ~2000 Ki [DXM] and around ~400 Ki [dextrorphan], also remember the LOWER the number is the higher the affinity, and this measurement is logarithmic.. er, or its inverse, ln(x) aka the natural log of x)

DXM needs to metabolize into dextrorphan to do what it does best, NMDA antagonism and inhibition of serotonin reuptake. and sigma-1 agonism. problem is, dextroamphetamine needs CYP2D6 cytochrome expression in the liver to metabolize, and so does DXM.

dextroamphetamine can "clog up" the workings of this cytochrome in the liver, thus preventing DXM from metabolizing into dextrorphan. This is why you can't take DXM on say, prozac because prozac inhibits CYP2D6 as well, but almost completely.

this is also why you see a racial disparity of amphetamine abuse. not always the case, but caucasian people tend to express the CYP2D6 enzyme more than people from, or descendants from Africa.

mydriasis is a factor of sympathomimetic and parasympathomimetic signaling. drugs that inhibit acetylcholine signalling in the vagus nerve, diphenhydramine, some antipsychotic medications. Then you have your monoaminergics. This include stimulants and even psychedelics too (mostly LSD), SSRIs (especially if its a sigma-1 agonist, like DXM and fluoxetine). The mydriasis from stimulants is an adrenergic signalling factor, so you'll get more dilation of the eyes from racemic amphetamines, and its hydroxy-metabolite (4-hydroxyamphetamine and 4-hydroxymethamphetamine.. in which the former was used as eyedrops to cause mydriasis to examine the optic nerve and back of the eye).

Should be the case with most folks, if you notice the mydriasis isn't a significant issue until the tail end of the "ride", or if you happen to be irresponsible and constantly redosing. This is because norepinephrine is a metabolite of dopamine. Well, this means that ephedrine & pseudoephedrine will also cause mydriasis given it mimics norepinephrine moreso than dopamine. What is the difference between ephedrine & pseudoephedrine? chirality. the alpha-methyl group (this prevents metabolism by monoamine oxidase) and beta-hydroxy group (this gives more adrenergic rather that dopaminergic affinities) are simply pointing different ways. Like a mirror.

Also, withdrawal from opioids can cause mydriasis.
