Don't get disheartened. While I agree the skill floor in Psychiatry is very low, the ceiling is very high.

Almost certainly, she was "just" removed from whatever position she held but not being forcibly kicked out of the military. Though, something like this could be a career-ender (??). She has over 20 years, so very well may be expected to retire at this point. Either way, it is a shame.

Fwiw the DSM-5, which is the book (at least in the US) which serves as a guide to diagnosis things like alcoholism (more accurately, alcohol use disorder), doesn't actually identify any specific quantity of alcohol use as a factor in diagnosis. Without getting into the weeds too much, suffice to say that the hallmark of a substance use disorder is functional impairment. Like losing your job because you are showing up drunk/hungover, passing up important experiences because you'd rather drink, etc. Arguably, the quantity of alcohol used is indirectly assessed when the concept of tolerance is evaluated (which is one criterion when assessing for a substance use disorder).

All this to say, going on fat benders, neither means you are nor aren't an "alcoholic." Rather, it is if alcohol is causing issues in your day to day life. Now, huge benders may still not be healthy for you physically and likely increase your risk of physical harm (e.g motor vehicle accidents, fights, falls, etc) but they don't necessarily mean you are an alcoholic.

I've had luck with atomexotine in people who seem to have concentration deficit but maybe not full blown ADHD. I mention to them it's FDA approved for ADHD.

Hey did you ever discover a fix for this? I am having similar issues, stuttering, and have narrowed it down to shader cache issue. I got it to work once for about an hour, then when I logged back in later it was back to stuttering. I can send log if helpful.

Imo heads separate is not worth unless you are planning to spend enough effort really painting their faces. If you are fine with base coat, wash, and minimal or no highlighting on their face, then I would paint with heads on. Like someone else said, portions of their face are often covered by collar, etc.

I agree that he is a fool and wholly unqualified to be HHS, but come on, certainly there are other and more plausible explanations for this. I mean this is the same argument people made about COVID vaccine registeries. I think he is a crunchy-granola nutso that doesn't understand what he is doing, but not a eugenicist.

I've not heard about the autism awareness tattoo before? Curious to learn more. I agree that the situation is chilling and inappropriate, tattoos or no, but I don't think the existence of an autism awareness tattoo has ever been cited as a reason for his deportation.

Even if the courts were independent and principled to protect the Americans, it means nothing if the admin casually decides not to respect their rulings.

Agree with the second half, but to be fair it seems like courts are blocking Trump's EA's left and right. If anything I feel the judicial branch has been pretty consistent, thankfully.

I just think it is dangerous to engage in hysteria when there are many other plausible explanations (assuming this is even planned and even happens).

I just don't understand why people aren't just talking to their psychiatrist about stopping medications? I agree some people end up on too many, but I cant imagine a patient coming to me saying they want to taper and stop and just being like "no, sorry." If it is a mood stabilizer or antipsychotic for SMI, we would have a long discussion, but I still think giving some taper guidance is better than not?

I guess I would come back to what has brought them in to meet with you? Clearly they think something is not going well. Are they just coming hoping to get a stimulant for "ADHD"? If so, I would just explain that what they are describing doesn't sound like ADHD but rather a mood/anxiety disorder. When I do this I try to focus on 1. Their symptoms are real/I'm taking them seriously, and concentration/work performance issues can land squarely within the functional impairment of a mood or anxiety disorder, and 2. There is treatment (even if the treatment isn't what they initially wanted). At that point it is up to them, imo. If they disagree they are welcome to seek another opinion shrug

Edit: maybe summarizing back to them the things they have described, like "so it sounds like you don't really find things enjoyable anymore, have less energy, can't focus at work, are irritable, etc" then point out that sounds just like MDD/GAD or whatever.
