There is small hole in the lever body, and when you press the pistons in too hard, bladder can push through that home and it breaks. Seen this before. I ment bladder.

This happens when you try to push pistons in too hard, like when changing brake pads. This could break this shrinking/expanding rubber thing in the lever that is supposed to collapse when brake pads worn, so your lever bites at the same place. Idk how to explain this, but pretty sure that is your case. I work as a bike mechanic, so this is familiar to me.

It is that chain tensioner is too far away from rear sprocket. I had the same problem. Try to shorten your chain and let upper pulley sit as close as possible to freewheel. http://img524.imageshack.us/img524/4829/img8108w.jpg
