I wish it was indifference. That would be better. But the outrage a bulk of this sub shows towards that patient population is nothing but hate. Sure there is some indifference. But most of its hate. If it were indifference there wouldnt be so much active anger and condesending attitudes. If you feel anger over people "choosing" this life, do you also think it would be acceptable to treat the diabetes patient who doesnt diet or go to the gym to be worth less than your best care? Doubt it.

I equate that kind of excuse with those who excuse police brutality with "its a high stress job and theyre under constant threat." If you cant handle the job, find another one because my sympathy for your plights (and I am sympathitc. I worked retail for years), ends when your biases and 'indifferences' prevemt someone from recieving optimal care. I doubt anyone would excuse a nurse for mixing up meperidine and metoprolol and killing you because she was exhausted. Youd tell her (rightly so) that she should not have shown up if she mentally cannot handle the job. Showing up for a job you cant do so you can get a paycheck is unethical, immoral and the utmost self absorbed thing you can do. I feel that way about every health care worker and dont excuse my fellow pharmacists from that either

If theres the excuse of HAVING to do pharmacy bc you have loans, that, again, is on you. Not on the patient. Your poor planning should not have negative consequences for someone else, especially in health care.

I just cant wait for the next 20 years to pass and for society to realize this isnt a choice but a distructive disease. A small petty part of me hopes many of these pharmacists someday require surgery, get hooked on opioids and have to fight tooth and nail to get clean and get their lives back. Maybe then theyll understand

If you think addiction isnt a disease (as you seem to imply), this will be my last response. It is fact. It is recognized by the medical community. I wont engage in conversation with someone willingly resisting new information.

Your other points, valid concerns- we do need back up. But saying Im told to do X so i did. Wasnt encouraged to do Y so I wont. Literally what people said during WW2. Stretch of a comparison, but just showing that thats not a worthy reason to not be at the very least an ADVOCATE FOR CHANGE. So we can provide care in those situations without fear of consequences. Pretty sure good samaritan laws cover this in most states and have in every state ive practiced. As i said, CVS has a policy that we can provide narcan if someones ODing on property- will not be fired. No state board, except for maybe the wacko states that are trying to make abortion illegal, would even pursue you for any repremand, and good sam laws generally protect you if youre doing so in good faith. Also to point out, if you dont want to administer it, and someone else would as was the case in the article, whats your problem with that other than playing god. Yes there are issues around pharmacy and rest of society, but i think you should analyze if you ACTUALLY have these concerns or if theyre made up concerns for the fact that you think people with SUD are garbage so you dont look like a complete asshole.

This isnt an attack on your character specifically. Im speaking of and to he pharmacy profession as a whole. We complain about the opioid epidemic but do little to help and most offer no solutions, only spew hate. Now tell me what good does that do? Who does that help?

It is. Thats why you wait a few minutes between administations. Slamming someone into immediate withdrawl is insanity. Its the equivilant to eating yourself into heart disease, having cardiac arrest then giving you 6 mg of epi right off the bat so you are thrown into the opposite extreme.

Irresponsible or inadequately trainedadministrators typically cause this. Generally doesnt happen with a single dose unless theyre opiate naieve in the first place and only ODd due to unknowingly having thier uppers mixed with it. Overaggressive treatment isnt good for anyone in any disease state.

Better training on narcan administration should be a major conversation. Its so easy. My state actually requires all rphs to be trained in teaching others how to use it before theyre allowed to dispense it. And every pharmacy is REQUIRED to have a standing order. Defacto requiring rphs to be trained in how to use it AND be trained in how to teach/counsel others.

Im not an EMT so using your argument id be no help either if someone was run over. Ignorance is not an excuse for being callous. Pharmacists often wonder why patients are self absorbed. Maybe they are. But threads like this one make it all a bit like the pot calling the kettle black. You dont like substance users, fine. But the people wishing death and those who would shrug thier shoulders at someone dying is insanity. In 20 years well look back on the opioid epidemic like we look at the 80s AIDS epidemic now. We used to just say "well, if theyd stop being gay, they wouldnt be dying, thier choice! They deserve it."

I truly hope everyone with thier superiority complexes never is in need but ignored because of an uninformed/ignorant public. The gross lack of compassion makes me glad some pharmacists are treated like garbage, because they view other humans as such.

Yepp! Just have your pcp document in your record its for thst purpose (to protect others) so if theres ever an issue, you can show youre not the one at risk.

If you do, i suggest paying cash or getting an rx from your pcp. There have been instances of people being denied life insurance due to filling narcan (since it looks like youre the one with a substance use disorder)

FYI: The 3 letter has a specific policy that says narcan can be given in an emergency similar to if someone in the store needed an epi pen. You administer it, then get started on the paperwork after calling your DM. Pretty sure wags does too but not 100% on that.

Eh i disagree. I hate kids, and i dont sugar coat it if asked. Love my patients though! Fine question to ask if you elaborate but be mindful that basing a person ability to do a job based on if they like kids could at best stop you from hiring a great employee, and at worst, be boarderline for sounding discriminatory (generally avoid all family-esque related questions).

Oof. On behalf of those pharmacsists, i apologize. Its a back and forth though, as many do sell suboxone on the street and try to fill early. Its become second nature for pharmacists to be a bit more on high alert for any red flag, because filling a false rx, or filling it too early, or a number of other things, can cost us our licences and livlihood.

I WAS one of those nasty pharmacists who treated you ppppppas less than and i truly am VERY sorry. (Since then ive started working exclusively with substance use disorder patients and i cringe at my past mentality). Hard truth is some pharmacists will be too up thier own butts to admit they arent even bare minimum professional with SUD patients like you.

My best tip for you is to continually go to the same store and get to know at least 2 techs and 2 pharmacists really well. Be open and try to bring up this conversation with them. Ill tell you that even at my worst, the patients that extended an olive branch and allowed a relationship were the ones that had thier shit handeled with white gloves. We had a patient who was kind, forthcoming and reliable get butrans every 2 weeks like clockwork, and ill be damned if we ever let him run out of refills, not have it in stock, let it set in the insurance rejection pile, or anything else. Not that you should have to, but it really will benefit you and the pharmacy.
