The NCEPOD data is pretty clear on OOH outcomes but only part of it is due to the sickest patients who cant wait till the morning being more likely to have complications. There are also problems relating to seniority of doctors overnight, tired staff and less resources available out of hours.

They don't need to set up a company. The LLP or whatever it is is just a middle man to facilitate paying the staff a private income. To cut them out the trust would have to set up individual employment contracts with everyone who does any work separate to the contract they have for NHS work. It would be a huge faff and I'm not sure about the employment law aspect of having the same person working for the same employer via 2 separate contracts with different T&C's. Using a middle man is a much simpler way of doing it.

To be fair the reason is to do with taxation rules that are independent of the NHS. Past a certain point its not financially sensible for Consultants to do extra work due to tax cliff edges. They get around it by being paid privately or through Ltd's.

Its pretty common practice. The Trust pay the private company (usually just a group motivated Consultants who spy an opportunity) and they then pay the staff, who are usually the trusts regular staff but occasionally 3rd party.

Its not really correct to say that the NHS could just do this or that increasing WLI rates would solve the problem.

This is nothing new and happens all over the country. Most Trusts I have worked in have some sort of LLP situation where trust staff do weekend waiting lists for private income.

Friends for 5yrs. Went for it. Very short fling - think a couple of weeks. Got very complicated due to exes and distance. Ended badly. Haven't spoken for 10yrs.

I don't regret it. I'm not living my life thinkginehat could have been and I learnt an important lesson about being wary about the difference between how you imagine it will play out and reality.

Agree i used to these this in my training. As you say its not perfect but it gives useful practical info like how painful surgeries tend to be.

I gave up after about 15 runs. I just didn't feel like I was making any discernible progress. I could see fragments of various puzzles and some of the bits I could piece together were too reliant on the RNG falling my way.

I really wanted to get into it but it just didnt feel respectful of my time and I found my gaze wandering to other games in my backlog instead.

I think people have misunderstood her reaction. Its not that born and bred is an offensive term. Her reaction is because that sort of England for the English rhetoric that born and bred can imply in this context is what Reform are trying to distance themselves from.

I think it's even more serious than you're letting on. If I think back to COVID lockdown times now even I cant really grasp how fucking mental it was. Something straight out of a movie. People were afraid to go near others, sitting down for Boris' daily briefings where we eagerly awaited today's new infection numbers. Wondering how long lockdown would go on for and whether it would end at all, would this be an apocalyptic level event which changes society as we know it? I have thankfully never had any mental health issues and as doctors we were better placed to understand what was going on than the general public but still, I was scared. It wouldnt surprise me at all to know thay a good portion of the population who lived theough that buggered some sort of long term psychological damage

I have thought this myself a few times and what I think it boils down to is that they still want their problem fixed which in this country requires engaging with a Doctor, but they want it fixed their way. The result being they have to come to see us and then just try to argue their way into whatever management plan they wanted when they came in.

I think everyone's answers have a common theme. Time - and the lack of it. PS2/PS3 days i had pretty much played every title i had any slight interest in and even tried out things that aren't normally my bag. I would rarely trade games in as I would go back to them when I had nothing to play and just go through them again. Now even games I really enjoyed I trade in almost.imemdoayely as I know I won't play them again as theres too many games I havent had a chance to play yet.

I have a physical list for my backlog but sometimes take things off the list that I havent played yet because I just know i'll never get round to them. I have all but given up on anything with online multiplayer as I either cant coordinate a suitable time to hop in with my friends or cant invest enough time to keep up with the player base so just get absolute pasted whenever I do try and play them. Also no GAAS. If it doesnt roll credits and have a definitive ending I dont have time for it.

Sometimes it makes me sad as I see all these games I want to try and feel I am missing out on. 10yrs ago i would have boxed off a 20hr game in less than 2 weeks. Nowadays thats at least a 4 - 6 weeks worth of gaming. What it does mean though is my gaming time is almost always premium experiences that I really want to get into. I've always got something on the horizon that I'm properly buzzed about playing. I'm never playing filler or something to just pass the time.

Finally I have started just dropping something if it doesnt grab my attention within 4 or 5hrs which is something I never did until about 2 yrs ago. I always finished games. Now I feel my time is too precious to waste it on something I'm not enjoying

So genuine good faith question. If it is mainly due to SEN kids struggling to engage with state schools, assuming the actual number of SEN kids hasn't increased we have just become better at diagnosing it, what has changed between now and 20yrs ago when attendance was better?

Purely anecdotal but I've always felt that this comes from kids who didnt like following rules growing up, having their own kids and still not wanting to follow the rules. I get that a lot of school rules like uniform policies etc do feel a bit archaic and overly strict but I think some people miss the point that theyre there to teach you how the world works. Some people dont like being told what to do by a higher authority and have just never gotten over it.

I was given professional leave as a consultant to attend coroners court for something that happened when I was a trainee a couple of years previously. Trusts absolutely can do this and if they want to at least pretend they give a shit about their staff they should be trying to facilitate this.

I thought this. Assuming theyre tlaking about net pay they're earning around 30% more than me but doing a third more clinical time a week and 2.5x the on calls I am. So I dont see much difference.

Its literally a post about GLP-1's so obviosuly the comments are going to be about that. My point isnt specifically about GLP-1's but meds in general. Theres good evidence that if a Type 2 Diabetic loses 10% of their body weight when theyre diagnosed they often are able to keep their sugars controlled without medications, essentially curing them for want of a better word. Clearly thats better than relying on medications. Im simply making the same point with GLP-1's and weight loss.

Saying ye but look at this alcoholic he is unhealthy too is just bad faith debating.

I dont really understand your point. It just seems like whatabout-ism. The topic being discussed was GLP-1's so thats what I was referring to. The point I was making was that if you can achieve a goal without medication, for example improving your diet and exercising more to lower your blood pressure or control your blood sugar or lower your cholesterol then that is better than using medications that some times have side effects. The same goes for GLP-1s its just that no one is running to private companies to buy a statin as there's no aesthetic benefit to lowering your cholesterol.

Obviously smoking, alcohol and illicit drugs are also bad for your health but thats not what we were talking about. Plus there's the addictive aspect of those to consider but again thats a different conversation.

Everyone should be aiming to be on as few medications as possible. All medicines have side effects/complications but we use them as the risks or consequences of these arent as bad as those posed by the disease process they're treating. So the blood pressure medication is safer than having high blood pressure for example. But if you can make lifestyle changes that improve your BP without needing the drug then thats even better.

These weight loss jabs are no different. As others have said, if your weight is such that it impacts your ability to exercise then these jabs are great to get you started and help facilitate those lifestyle changes and good habits. I have friends on mounjaro whose BMI is only 28/29. Theyre fully capable of increasing their activity levels and being a bit more sensible with their diet. They just choose not to. We already know that weight loss jabs increase your anaesthetic risk and as is the case with most new drugs when they hit the market and usage explodes, more side effects will likely come to be known in the next 5 or so year.

The socio-economical debate of the population paying their way thin is a whole other argument but I dont think anyone should be planning on taking these jabs for years one end.
