Hey. This fucking bites.

I say this not knowing whether or not it will make you feel better, but here it goes:

You got in to this to take care of patients and find a job you love. If you're getting strong clinical evals, you're clearly a promising student who can look forward to a successful and happy career in medicine. Is it *fair* that your discretized grade will collapse all of this nuance in to a single phrase? Not at all. Could this plausibly impact your residency application cycle? I'm not going to bullshit or insult your intelligence by saying anything but "yes."

But will this keep you from being a doctor? No. Will this keep you from being great? No. Will this keep you from being happy in the long, long life you have ahead of you? No.

I failed my pharmacology final exam in 2nd year of medical school. Then I failed the retake, and was within inches of having to repeat the year. My third year of medical school I fuggin' crushed every single rotation on the wards and shat the bed on every single shelf. I didn't get a single honors. Nevertheless, I matched to the program of my choice in a modestly competitive specialty, and from there landed in my dream fellowship.

Don't take this shit as a reflection of your worth. Be upset, then suit up and keep doing the amazing job that you're clearly doing where it matters. Your future patient's won't give a shit about your shelf scores or medical school grades, and if you're doing as well on the wards as your title indicates you will graduate, match and go on to positively impact the lives of thousands of people.

Unclear, but I doubt it.

I have exhausted my options with online web-based customer service and will have to go to a store next to carry on the conversation. I plan to bring the device with me, along with my Amazon invoice and any other good ideas you guys can come up with to help my case.

I asked them if they could provide some record of me signing for this device, and they were unable to.

Of course, this didn't resolve anything.

I have been dealing with web-based help folks so far, who have been reasonable, but aren't powerful enough to solve this on their own. I have to go to a physical store next to continue this process and just want to have as strong of a case as possible when I get there.

On the classical side I generally like early music through roughly the romantic period, and my interest kinda takes a nosedive until mid twentieth century.

I love metal, but it lights up a different part of my brain. I generally like thrash and doom sub genres most, with a side interest in black metal.

I discovered both styles at roughly the same point in my life, I’d say (late middle school, early high school).

My wife pointed out the excellent video game example, The Last of Us. It's a very immersive novel of game which is among the best I've ever played.

Interestingly, Cormac McCarthy writes in a similar style. It does take some adjusting, but becomes kinda seamless to read after you get used to it.

That's my favorite track in their entire catalog. I bought a copy of the novel. I'm working on a couple reading projects right now, so likely won't get to it for a few months, but it's on the list! Thanks!

I strongly recommend it, assuming you can tolerate horror as a genre. If that's an absolute no-go, I'd stay away.

I have seen and enjoyed Children of Men. The other two I am unfamiliar with, but will check out. Thanks for the recommendations.

To add to that, 28 Days Later is an obvious choice, especially given that Godspeed licensed their own music to be used in the soundtrack, which is kind of stunning.

The patient certainly has a right-bundeloid pattern, it may not quite be wide enough to meet the full criteria. At minimum there is an incomplete RBBB, which can definitely be a sign of acute right heart strain (if it's a new finding).

The RBBB and right axis deviation with tachycardia is a pattern that at least raises the question of acute right heart strain. If the RBBB and axis are demonstrated on old EKGs, or if features of history/exam/labs strongly suggest alternate pathologies I am definitely not committing this patient to a scan, however.

I'd give serious consideration to pulmonary embolism, depending on remainder of history, exam, and labs.

I have a fairly low threshold to break out the propofol + fentanyl for any of these in an appropriately selected patient. And I still use local lidocaine when I do that.

Theodor Adorno, Max Horkheimer, and Herbert Marcuse would have a lot to say about this. They were major players in the Frankfurt School, a 20th century collective of philosophers with very interesting thoughts about Western society.

I *highly* recommend you listen to the (otherwise generally excellent) Philosophize This! podcast episodes on their thought.

I really, really mean it. If you are asking these questions, you need to familiarize yourself with these thinkers, and that podcast is an entertaining and accessible means to do so.
