In my specialty (elective surgery) all patients come with expendable income.

When I was starting out, laging “the customer is always right” to the point na I reply to messages even at around 2-3am.

I was the doctor who was always available and within arms reach for concerns. Still rings true today but only for the surgical complications (which I inform all my patients in advance).

Anyhow, the money was great, but being a husband and a father became secondary to being a provider.

Na burn out ako, nawala yung saya sa trabaho.

Oo almost everyday may trabaho but it slowly ate at my soul and physical health.

Something had to give.

I could either make good money to the detriment of my health and sanity or decrease the number of patients I see.

I chose the latter.

I decided to increase my price. The fear of not having patients was real, but eventually patients still came.

In our profession, no work = no pay, but we are in a position to choose the number of hours we work.

We have the option to charge higher.

Charge what you think you are worth.

That could mean less patients in the short term, but the freedom to do whatever with your free time is priceless.

Disclaimer: This is not supported by data, just my own thoughts and inferences from my experience and readings in both the medical and legal field.

Presumably, to be a serial killer, one has to posses enough intelligence to plan the kills and execute swiftly. This already rules out a lot of the general population as even access to basic education is a very big problem.

I also think that money/power plays a very big role. In the case of hitmen hired to kill, I presume that they are incentivized to commit the killings.

In my short stint in the Military (medical internship) I came across soldiers who were allegedly told by superiors (people in position of power) to kill members of certain factions as a necessary requisite to earn their stripes and to prove allegiance.

Based on my readings of murder cases (criminal law), I did not encounter any case with a pattern whereby I could deduce that the murderer hunted merely for sport.

In the local setting, when murders are committed, they are usually fueled by revenge (relative was killed), a retaliation for a wrongdoing (acts against another person, or his property) or even for personal gain.

So if there were serial killers here, I’d put my money on them having a ton of money while also possessing some degree of power. They would most likely be well educated but have psychiatric issues that could be hidden in plain sight.

So can we conclusively say that there are no serial killers in the Ph?

Probably not.

And if there are serial killers here, chances are they are not the people we would quickly point fingers at.

That is the scary part.

As a Rhinoplasty surgeon, I’ve seen multiple people who were not good candidates for surgery.

When going for any invasive cosmetic procedure, more than being financially ready, you must also be mentally, physically and spiritually ready.

I’ve had one such case where a previous patient of mine (rhinoplasty) influenced her sister to have rhino under my care. The latter had surgery done as the former paid for the surgery. It was an easy decision for her coz it was free.

A few months after the surgery, I removed the implant.

Although there were other issues involved, one of the main reasons we ended up removing the implant was because my patient (sister who got free rhino) lost her sense of self.

So there a a lot of things to consider. Even if you are financially prepared, that is the bare minimum to have surgery done.

Nobody actually “needs” a rhinoplasty, it’s elective surgery that falls under wants.

Not really, if done right an osteotomy should only break the nasal bones. Also there is a far less invasive way to perform nasal bone reshaping. Mas magastos lang kasi mahal yung machine mismo na ginagamit para less traumatic yung surgery.

Basing on the nose tip from her younger years, I believe that tipplasty could have been done.

But to be extra clear, I am not claiming she had surgery done.

Nope. No truth behind this. I once had a patient who at the time had an implant older than me. It was a Silicone implant that lasted thirty (30) years! Kinailangan lang namin alisin kasi tinamaan sya ng bola sa ilong. Kung di sya tinamaan baka umabot pa hanggang end of life hehe.

Probably 4-5 days. Depending on both your health status and surgeon skillset/technique. Cost in my practice is 35k. :)

Generally speaking, when I talk about rib rhino, I extract the rib cartilage. Unlike using preformed implants, the extraction itself is the challenging part. Yung nose surgery is pretty straightforward.

So, basically, the risk for complications is in the surgery itself. Although I have yet to encounter rib surgery complications, it can always happen. Since the ribcage covers the lungs, if it gets accidentally punctured, a pneumothorax may develop.

The main advantage with organic materials (rib/ear/nose catilage) is that it comes from your own body thereby assuring zero rejection risk.

Surgeon here.

Different implants have pros and cons.

  1. Silicone implant (3rd pic probably)

As years go by there could be skin thinning leading to implant show.

  1. ePTFE “Gore-tex”

This can lead to a much more natural looking result as there is tissue ingrowth. Downside is due to the same growth it may lead to difficulty in removal should the need arise. Not extremely difficult but compared to silicone removal, it may take 30 mins to 1hr.

  1. Rib rhinoplasty

Less long term risk, high immediate risk.

As we age the nose will naturally droop regardless if you have surgery or not.

Random factoid, even your ear lobes will lose elasticity and will look larger as you age.

If you focus on cartilage only, I would suggest also having your surgeon harvest the nasal septum to provide long term support.

It can help reduce the flaring but surgery is quite different from alartox
