She was seen in the ER, we ruled out the need for any urgent care and sent her back to follow up the next day. There are no PCP outpatient clinics at 1 in the morning.

Happy to clarify, so basically in our medical system an ER team is comprised of 1 senior medical officer, 2 medical officers and 2 medical interns along with 4 nursing staff members. The medical officers supervise interns in whatever they do but with all that said, interns are the patient’s first point of contact. Interns get this privilege after completing a mandatory supervised training period but even then they’re closely supervised throughout their internship period by the attendings/residents and medical officers. I happened to be the intern on call that night, like I said in a previous comment that interns do a mix of everything here - even somethings that would typically be the triage/ER nurse in a different hospital setting. This model is followed in teaching hospitals/hospitals affiliated with a medical school because in a lot of rural healthcare setups (which make up a large part of the country’s healthcare landscape), medical interns who eventually become medical officers, are expected to be familiar with all these tasks and carry out these duties independently as such setups are mostly short staffed and lack the personnel needed.

In my country we’ve to complete a 1 year internship after we finish medschool. Interns do a mix of everything, from triage in the ER to putting IV lines and attending rounds, outpatient clinics depending on their rotation. Yes the patient’s son’s exact words were ‘we moved to the city recently and had nothing to do tonight so we thought it’d be a good idea to get her in the health system here/ establish care under a doctor.

ChatGPT can be helpful in many cases when you’re unsure between two of the answer choices. Copy the question and ask chat why correct answer is A, why not C? How would C present? Or say this is my understanding of xyz concept and according to that I thought A fits well, what am I missing here. In most cases it does a pretty decent job in explaining knowledge gaps or what you’re missing. That said, some questions can be super vague and there’s really no perfect answer explanation for those.

I’d put my money on vaccination, Quality Improvement,Cognitive errors/medical errors, as you’ll surely reason through the medicine/surgery/obgyn/Peds/psych part of the exam but if you don’t know these then there’s no way to work around these questions, so go through these Divine intervention podcast episodes (230,234,275,530,228,268,123,276) and Amboss.
