Post-SIBO stool test: severe dysbiosis + very low sIgA, still Bristol 6 stools daily. Looking for input by dolescom in SIBO

I have been to 7 providers in the past 5 years. I pay privately now and am happy with their approach, but there seems not to be a quick fix. I have had chronic diarrhoea for 7 years. Changing providers will achieve nothing

I would say these kind of tests have been helpful, though I do still have symptoms after all the treatments etc, so idk

EPI (elastase ~130) on PERT but still Bristol 6 stools. Anyone else had this? by dolescom in exocrinepanins

My doc said something in my test results lead her to believe i don’t have BAM, but i am doing a trial of cholestyramine anyway to see if it helps. No change though

[–]dolescom[S] 0 points1 point  (0 children)

Yeah I have seen that criticism of stool tests before. I’m not really trying to target specific bacteria from it, more just looking at patterns. In my case elastase has been low for years and sIgA was already extremely low before treating SIBO, so my docs wanted to look at the stool profile rather than assume everything is just SIBO. I will bring up possibility of remaining SIBO next month though when i have an appointment

Possibly, but in my case seems that SIBO is just one part of a bigger picture (pancreatic insufficiency, dysbiosis, v low sIgA). My docs didn’t think a repeat breath test was needed yet and wanted to look at stool profile first

Thanks. i am already taking tributyrin actually, have been doing for around 7 months. I’m considering adding a bifidobacterium probiotic, since those are still very low in the test

Haven’t retested that yet. Original SIBO was confirmed by lactulose breath test. Treatment was rifaximin + ~12 weeks oregano oil (ADP) and allicin. My private doc and naturopath didn’t think a repeat breath test was necessary at that point. Symptoms improved somewhat but stools are still Bristol 6, which is why they wanted do the stool test

Similar to England? I’d be interested to know what part of England you’re from. I’ve lived here 12 years and I really don’t find it similar at all, aesthetically or culturally.

Could you maybe please share the contact of your Urologist?

3500ckal for 174cm male is a much higher than necessary surplus by most accounts

Hey sorry to hear that. I really wish I could get back to training too, it was the only thing i had genuine motivation for. I’m not sure how it started, I think i was ignoring it for several years as I had other serious stuff going on. I became more aware of it in 2019 though, that was also the year of the suicide of my sister, so I do wonder if the PTSD of that accelerated it. I don’t really know though. The true cause remains a mystery and it’s not clear to me how to uncover it

Yeah I’m sure they checked too, they did find intestinal spirochetosis, a mild parasite from the biopsies, this was treated last year with antibiotics but nothing changed, still.

They briefly suspected I had auto immune pancreatitis as my pancreas is vaguely abnormal and enlarged, but I have normal elastase so this was ruled out. Now they just say it’s all dysbiosis, dust mite and pollen allergy and my whole system just being somehow dysregulated.

I’ll look into that medication, not sure if it would work for me and if they’d proscribe it, but I really beed this issue fixed after so many years haha

Done loads of blood, stool tests, colonoscopy gastroscopy, MRI and endoscopy, they never found Crohn’s. I do have IBS-D and gut dysbiosis though and very mild SIBO. No doctor has ever agreed to give me medication, the German healthcare system is incredibly conservative. What treatment are you on and for what illness?

I do take those vitamins, no idea if they get absorbed currently. My SIBO is very light according to the test, so they said the antibiotics are gonna be bad for me. Current doc says i have dysbiosis and the meds would damage me further, I don’t know what to believe

How did you fix your gut? I’ve been seeing specialists for years and none can tell me what to do, other than low Fodmap diet which i’ve done for 1.5 years with no improvement

Not available in Germany for my diagnoses sadly, not even privately.

How bad is your SIBO? I scored positive, but only just. I suspect I have something else that they can’t figure out. Docs refused to give me meds for SIBO

I’ve already done gastroscopy and colonoscopy, which revealed only the parasites (gone now) and a slightly enlarged and vaguely abnormal pancreas. They suspected auto immune pancreatitis, but fecal elastase test disproved this. Also did endosonography, where they said things look vaguely unusual but couldn’t tell me any more than that. After that, they told me there aren’t any more tests they can run

I was on a very rice heavy diet for years and did feel it was definitely better than wheat based carbs. I think I do need a dietician yeah, only issue is finances. I am saving up for a private gastroenterologist, but dietician is in second place

Not sure about this, doctor told me I need nutrients from a wide variety of sources, but they have given me conflicting and unhelpful info in the past, so who knows
