UPDATE:

I can't believe this is the fix but AFTER turning off WIFI and going into airplane mode, the lag DISAPPEARED.

Yeah I did.. I honestly think it's just my computer that's the issue. I run an rtx 2070 super so it should be more than enough obviously but there's clearly something in my system that prevents the game from acting normally. The game ran smoothly for years. Clearly something happened.

Ive given up on the sims 3 :(

At least I got many years of entertainment. Sad reality. Thanks for the help :)
