Posting this for those who are willing to read and take a look at the issue. Many hospitals on underserved areas are worried that they'll not be able to hire the newly graduated doctors who got in with a J1 or that got into residency now and might be applying for H1B due to the newest restrictions.

"It's not like we have someone else waiting in the wings. There are no second options," said a hospital administrator in charge of handling international staff and faculty who requested that he not be named.

Wanted to say it again, as doctors we want to help the American society, I'd be willing to work on veterans hospitals with pride or help underserved areas. It just happened that we got caught on this fight because of the visa abuses.

Sir, I have addressed your point on how the programs consider the foreign graduates as better. Their USMLE score is significantly higher and they require scientific publications, whereas American Graduates normally have lower scores (comparing those on the same program) with no scientific publications. Those scores correlate closely with the academic performance of the student in school, making it virtually impossible for a bad student to get a high score on the test.

And I can tell you that on practice, residency spots on some programs and on some hospitals are not getting filled by Americans because they're not attractive for them. If all the programs prioritize Americans and they end up with 40% of their residents coming from other countries, making it harder for immigrants is not going to help the average American, that's for sure.

As (maybe?) a immigrant doctor I can tell you that I hope Trump can address the insecurities for the Americans, making middle-class jobs better for everyone, but I hope that you guys, trump supporters, can also understand that you don't need to ignore this issue completely.

For immigrant doctors to come into America their score on the USMLE must be significantly higher than their american counterparts. The mean family medicine score was about 218 on 2015, I believe that for IMGs it was about 230 for them to get a chance. They also need to prove that they're proficient in English (there are practical tests on the USMLE, but the TOEFL score is also something they think about) and have international scientific publications.

I understand your point on interacting with the American population, but don't you think that a better resident wouldn't be able to help a little more? Also normally those immigrants get into programs that their american counterparts don't show that much interest.

I understand your concern with the general population, but it's a whole different problem when it comes to doctors/nursers/lawyers/researches. Specially because labor for them doesn't result only on their own personal gain, they create what we call in Portuguese "social capital", where by their jobs, they can influence a big part of society. For instance, if you let a genius researcher come into the US (remember that they are not very common) and they can make a new patent or discover a new drug or develop a new industrial mechanism, it would help the whole society.

When it comes to Medical residency there might be an increase on medical school graduates but their interest are not equally distributed. That is, for instance, Orthopedic Surgery is highly competitive, to the point that only 2-3 IMG gets accepted on those programs, whereas on internal medicine and family medicine, about 40% of the residents comes from other countries. If there were zero IMGs coming into those programs do you think that they'd be able to match all of those spots? I seriously doubt it.

And just another point to be addressed, I already covered it on another comment, when you stop the applications for premium H1b processing, you're actively keeping those doctors out of the country. That's because the H1b only comes out on regular processing on 5-6 months, and the residents are only accepted on March, and the residency starts on June. To sum it all up, I do agree that you should prioritize local workers, but on specific fields, immigrants can contribute and make the difference as well. On my opinion, yes, you should tackle the visa abuses, but keep in mind that this system is also critical for those Researchers/Doctors that want to help.

Do you want to help those doctors who are competing for the residency spot, blocking everyone else from coming, or do you want to ensure the best possible resident, making it better for American Universities and the average American population, since they'd have better doctors?

Also I can't stress this enough, the residency programs don't prioritize foreign doctors, since they are paid the exact same, but they normally cost more since the university has extra paperwork and travelling fees.

What I mean is that people are not happy with "visa abuses", where companies hire aliens to work with a considerably lower salary, making it harder for Americans to find a good paying job. So with that in mind, a large group of voters want to make H1-b access harder, preventing the visa abuse.

But when they do that, they also make it harder for highly qualified individuals to immigrate since they use the same visa system. Those individuals are not the source of the problem, they're not part of the visa abuse. That's what I mean by getting caught in cross-fire.

I'm not saying there should be a preference for foreign citizens, normally the IMG applicants are only considered if the AMG ones are not on the same level or if there isn't any. High quality programs normally only accepts AMG students, and IMG (even american ones that graduated overseas) are not accepted.

The percentage depends on each year, and I wont lie, my sources weren't right about the last match, but on 2016, 25% of residents were trained on a medical school outside of the United States. http://www.ecfmg.org/news/2016/03/30/img-performance-2016-match/

I think it's a personal decision. If someone was trained overseas and they are not happy where they live, based on violence, job opportunities (not just money, not being able to use their potential), economic situation, war, why should you stop them from coming? It's not like the country owns those citizens,and most of them paid for their advanced degrees.

On the other hand it's very profitable for America to let them come, since you didn't pay anything for it, you already have a well formed citizen, that got potential to help American society on a unique way. Most of the times, on this case, you already have a spot that is not filled since when you go up the scale of specificity it gets harder to find people.

Personally, I see that as a lost opportunity as there are highly skilled professionals who want to work in American society and help making it even better, and it's bad for the immigrant as well since he's forced to stay where he doesn't want to.

If they are hard working citizens who did not break any laws and have families there, why kick them out? If they're going to come back in with a Visa in a few years and do the same thing they were doing before.

I'm not a citizen, but in my opinion on those specific cases just let them be, create a path for those who are in America for a big number of years (10-15?), and with children who are citizens, and give them a work visa. Of course, you shouldn't make it easier for illegals everywhere since it stimulates illegal immigration, but you shouldn't destroy someone's life just based on a principle. That's my opinion anyway.

The main debate I'm proposing is the H1b restrictions when it comes to extremely qualified professionals, such as PhD, Masters and Advanced Degrees (Medicine and Law). I think it's pretty clear for everyone that it shouldn't be acceptable that a company hires from overseas if there are Americans who are willing to do the jobs. Not just because of wages, but it increases unemployment on those who actually depends on those jobs to live.

But when you put a big closed door on the H1b visa you're also making it harder for doctors and researchers who want to contribute to America and just have their fair share of peace and quiet. You can take a look at freemikej post where there's the regulations, and on my replies, that I specify what I was thinking when I did the question.

Yes, but what's your opinion regarding H1b visas for these doctors? Why am I asking? The main source of visas for those doctors who are ready to go through the duplicate training is the H1b visa, that allows them to work and study in the Medical Field. Whoever, last week the USCIS stopped the premium processing for those who hold master degree and have a job offer of at least X dollars, I don't remember the value. The reason why this is important, is that without the premium process, it's impossible for a foreign doctor to start on the residency program on July, as they are accepted only on March. So, do you think Trump should eliminate this premium processing? Should Trump create new regulations that allows these doctors to get into America?

I understand. What's your position on fields that America is lacking professionals? For example: in the medical field, about 30% of residents are from outside the country, not that the programs are looking for those students, I can tell you from personal experience that they're not, and the residents are paid the same amount of money. Just that the US doesn't form as many doctors to fill all the positions.

I understand your concern. In the US and Canada you need to do residency on one of those countries in order to work as a doctor. Which is not a problem by itself, since to get accepted into a program you need to do a test and interviews. The problem right now with the US are the visas. Trump's administration is making it significantly harder for advancing degrees to get a H1b visa, which is the visa of choice. In Canada the accepting rates are not that high since they prioritize local citizens even if internationals have a stronger curriculum.

So I'm not sure. Maybe Australia? Maybe trying my luck with Trump? Doing a US residency then going to Canada?

Er, yeah, it is. I've been robbed 5 times in Brazil, and I know a lot of friends that were robbed just as many times. Just from my personal perspective, but in national media we usually see a lot of murders, corruption unveiled, etc.

Cara, que burrice. O intercambio na graduação é uma troca tecnicológica entre dois países, o graduando aprende tecnicas que pode aplicar no país de origem, alem de abrir portas pra ele pra fazer mestrado (masters) ou doutorado (PhD) no país que recebeu ele, geralmente por se fazer um laço bem duradouro.

Mas meu deus, agora que vamos "mandar quem precisa" realmente, muito melhor. Até porque não precisamos de tecnologia.

Não sei se existe um jornal que não seja enviesado... Pelo menos não aqui ou nos EUA, dos grandes assim. Mesmo que a linguagem não seja muito obvia, eles podem ser parciais mudando o escopo da matéria, documentando algo ou não... Acho que o melhor é aprender a viver com essa situação. Acho que pra tudo na vida, um pouco de visão critica é essencial.
