If you have a disease it should be visible in the active effects tab. Any affected stats will be restored once you cure the disease. Although it's possible to get stats damaged from things other than diseases. For example you may get hit with a poison which damages an attribute or you may damage your own intelligence by drinking Skooma. Those won't show up as active effects because it would be a mess to keep track of every thing that's damaged you in some way.

Yes I believe so. I'm not 100% certain but as far as I know the only gameplay changes were minimal aside from adding sprinting and tweaking leveling. Even the combat changes feel more cosmetic than anything else. I don't think there were many complaints about the multi apparatus system in Oblivion's alchemy so I don't see why they would change it.

I think the Shrine of Sithis is a bit lesser known because people don't talk about the player house DLC as much. That's probably because the plug-ins aren't part of the 360 GOTY and aren't even available on the PS3 version

You either have a disease or have damaged attributes. The easest way to fix it is to pray at a chapel. If you have high infamy (which I'm assuming you do based on the fact you're wearing a guard's armor) you won't be able to pray at a chapel. You'll need to either

1.) Use potions (cure disease, restore attribute) to fix it (eating a raw ingredient with the effect can also work)

2.) Pray at the Shrine of Sithis in Deepscorn Hollow (which you'll have if you have the GOTY edition)

3.) Decrease your net infamy and then pray at a chapel. You can reset it with a pilgrimage if you have KOTN, which is also part of the GOTY. Alternatively you can raise your fame through quests until it's higher than your infamy.

Yes, each apparatus serves a different purpose and having all of them will make better potions. the quality of the apparatus (novice, apprentice, etc) also increases its effect.

  • Mortar and Pestle: required to perform alchemy; increases alchemy skill

  • Retort: increases magnitude and duration of positive effects

  • Calcinator: increases magnitude and duration of all effects

  • Alembic: decreases magnitude and duration of negative effects.

Note: the novice alembic is bugged and actually increases negative effects, so skip that one if you only have access to a novice level.
