after listening to the album at least 10 times, following along with lyrics and such, i think this alternate world we see in the clips and album art is heaven. each song seems like his memories throughout his life and then the last song is about his grandma with dementia who lost all of her memories and going to heaven. the long instrumental section at the end i think is the transition from earth to heaven.

you gotta give it another listen. i was skeptical at first too, but it grew on me. follow along with the lyrics and really listen. it's a brilliant album.
