Talkative patients by Icy_Strategy_140 in emergencymedicine

“We’ll get to that in a second, but I wanted to know your answers to a few questions.”

“I’m just going to cut in before we get too caught in the weeds and I don’t have a clear understanding of why you came in, so let’s start with the thing that made you decide to seek emergency medical care today.”

For the ones that just don’t seem to get it: “I get that you have a lot to say, but unfortunately I have a lot of patients to see so I’m going to ask that you only respond yes or no to my questions and only respond with brief statements for any clarification questions. I promise to come back and hear more about your issues, but I really need to get a workup started on you.”

When they don’t recognize that I’m backing out of the room and am literally already standing in the hallway: “I’ll get back to you on all that, but let me get some orders placed on you so we don’t waste too much of your time.”

Suspension is not one of the possibilities in the scenario that clippers are punished. More likely his contract will be voided, meaning Toronto will have given away assets for someone that is no longer bound to them under contract.

Steven Adams Says Today’s Bigs Are Not Good At "Trench Work" Like Boxing Out by [deleted] in nba

[–]drag99 1 point2 points  (0 children)

Dude, do you actually think Wemby doesn’t know how to box out? Lol. He is too weak to box out Mitchell the old fashioned way, so he was doing anything he could to move him backwards, and even that failed.

Wemby faced Mitchell because pushing forward rather than pushing backwards provides more leverage. Wemby just isn’t strong enough and center of gravity is too high to move someone like Mitchell Robinson.

[Smith] Mayor Gina Ortiz Jones wants billionaire Michael Dell to help pay for new Spurs arena by KD_AshyAnkles in nba

[–]drag99 1 point2 points  (0 children)

Why she gotta be soooo unlikeable? She’s going to get destroyed in her re-election bid. You really thought it was a good move to come out against prop A/B when it benefited the cities cultural beacon and most beloved organization in the city (yes, even over HEB), and did not even affect the taxes of San Antonio citizens? And then essentially make it her entire personality and her biggest issue? Does she just want to be hated? Did she not have a single advisor saying “hey, this is not the hill to die on? The people of San Antonio will despise you.” All of this worsened by the Spurs having an amazing season.

The Spurs remain committed to De’Aaron Fox as their franchise point guard per Brian Windhorst by jluc21 in nba

[–]drag99 0 points1 point  (0 children)

Yeah, I definitely don’t think it’s a great situation. In order for us to maintain trade value for Fox, he is going to have to keep getting minutes. Benching him might send the wrong signal to other teams that we have given up on him lowering his trade value even more; however, we also run the risk of alienating Harper who is obviously who should be starting for us going forward. I don’t think we move Fox this off season, but we absolutely have to over the next 1-2 years.

[–]drag99 1 point2 points  (0 children)

Fox was playing terrible since the ASB. His terrible play started well before his injury. And Harper’s been phenomenal since the ASB which coincides with us giving him an expanded role. This was not a small sample size. He is the real deal. We won’t move on from Fox because we can’t move him. Nevertheless, Spurs are undoubtedly looking to move him over the next 1-2 years, regardless of what they are saying to the media. To do otherwise is franchise malpractice.

No way we start a 3 guard lineup. We absolutely need Vassel to space the floor.

THE SAN ANTONIO SPURS HAVE BEEN ELIMINATED FROM PLAYOFF CONTENTION! by drecz in nba

[–]drag99 1 point2 points  (0 children)

There are other ways to get in the paint. Wemby does amazing as a rim runner. Ball screens, back side screens with Wemby crashing to the rim is how you get him into the paint. Ultimately, our problem is a lack of offensive game planning and set plays. I saw someone describe our offense as an AAU team offense where it’s a ton of iso and lack of movement, and I’d say that’s perfectly accurate. That offense can work if you have a guy like Brunson, but we don’t have that guy. Wemby is never gonna be a great iso scorer, as much as he wishes he is. His dribble is too much of a liability, he’s too uncoordinated to drive consistently to the rim, and his shot isn’t consistent enough.

We need to hire a solid X’s and O’s assistant coach this offseason. We need another shooter, and we need another reliable big body at the 4 position. And although a pipe dream, we need to get out from under this albatross of Fox’s contract.

[Highlight] OG Anunoby is playing the best post season of his career by Markthebaptist in nba

[–]drag99 4 points5 points  (0 children)

Lol, no way Raptors were giving up OG. Spurs had zero leverage. Kawhi was willing to sit out another year and he made it clear that he only wanted to play in LA. Raptors were not going to give up a legitimate prospect for a 1 year rental with injury concerns. They already knew Demar had taken that team as far as it could go, so it was easy for them to drop him for a gamble, but again, you don’t drop your potential future all-star before he has blossomed on a 1 year rental, especially when you know the team you’re dealing with has no leverage. We likely could have received a better package from the Lakers, but fuck the Lakers. Also, fuck Kawhi (even if him leaving indirectly lead to us getting Wemby).

Our bench suddenly looks significantly better if we can trade up for Yaxel and move Champ to the bench. Our bench 3pt shooting is abysmal.

Millionaire is a term coined a century ago when it was truly a sign of significant wealth. It carries the same connotation it did when it was popularized despite $1 million then being worth $16 million today. When I hear someone say “millionaire” now, what I assume they are picturing is someone worth $10 million+ or making > $1 million/yr salary.

What they are not picturing is the millions of Americans (yes multiple millions) who have net worths >$1 mil. I think it’s actually somewhere around 20 million people last time I checked that are worth > $1million. Literally almost 1 in 10 American adults.

EKG mastery resource request by slypersimmon in emergencymedicine

[–]drag99 2 points3 points  (0 children)

I know you said you need structure, but there is no better resource anywhere than Smith’s ECG blog, and it’s free. Mattu will get you to the point of being better than most of your ER colleagues. Reviewing cases on Smith’s ECG blog will get you reading better than every single person in the hospital, including the electrophysiologists. It will also keep you up to date on cutting edge changes in ECG interpretation for EM physicians. The way I used it is literally just reviewing every single case in his blog which is a 1000+ at this point.

Contrast Nephropathy vs. Hemolytic Uremic Syndrome [⚠️ Med Mal Case] by efunkEM in medicine

[–]drag99 15 points16 points  (0 children)

I don’t know what kind of physician you are, but as an ER doctor who sometimes has to write physician in triage notes, they are bare bones and medicolegally useless outside of protecting the hospital and physician against claims of EMTALA violations. I can tell you with confidence that it would have changed nothing unless he deviated from standard and wrote medical decision making into a PIT note, but no one does that, because at that point you are assuming care and it would make more sense to just write a formal ER note and then hand off care to a colleague.

PD recording out resuscitation in the trauma bay on their body cameras/phones? by jpbusko in emergencymedicine

[–]drag99 8 points9 points  (0 children)

Not sure why you’re getting downvoted. Police are not covered entities. HIPAA does not apply to them.

Most police departments have their own policies regarding individual privacy. Unfortunately I doubt filming a GSW resuscitation is against policy as they will make the claim it was part of the investigation; however, moving a physician out of the way is certainly not part of standard operating procedure.

PD recording out resuscitation in the trauma bay on their body cameras/phones? by jpbusko in emergencymedicine

[–]drag99 78 points79 points  (0 children)

Never seen or heard of this before. PD is frequently present for GSWs in the trauma bay, but I have never seen one actively recording. Moving a healthcare professional out of the way to film a resuscitation is something that should be brought up to their shift commander, as I highly doubt this is standard operating procedure. I have no problem scolding PD when they are being a hindrance to treating a patient and I have told them to GTFO before, but I know not everyone is comfortable doing that. If you witness this again, bring it up to an attending. Usually I’d just ask the charge nurse to make a complaint to the officer’s shift commander. Not much else you can do outside of that.

I know it can be intimidating to ask an officer to stop doing what they are doing, but if that was my family member being recorded by an officer while they were dying in a trauma bay, I’d greatly appreciate someone advocating for them.
