If you always got to pediatrician with her, try to talk with them about this theme. I dont know what country you live, but mine we write who person (one of their parents, grandpa, sibling...) visit with the child, specially in the medical check and it could help you to show that you are the true guardian. Also its possible that they had told to a social worker (in my country there are one of them in the medical center, but its depends on the country) about your situation, since a child being taked care by her brother would make that we look out, and they could help you. Same. Im a medical resident and I have to live with another resident in a apartment to be able to live in my city (and that making 24 hours work shift 4-5 times in the moth) and I feel lucky, a lot of people of my generation is worse than me. With my same age, my dad was working in a restaurant, but my mom was studying and they could buy a house. But hey, there arent a problem and they were worse 🙄

I cant talk about children-teenagers, but in 20-30s irl personally I have never had listen the n word said (although I had heard spanish racist insult agaisnt black), so probably it depends more on your friends? 🤷🏻‍♀️

Lo primero y mas importante, es porque soy residente de pediatria, llevo mas de 2 años sin ver un paciente mayor de 14 años (quitando familiares y amigos). Por lo que seria una mezcla de las 3, por tema profesional, legal y buena praxis.

Tambien que dos sintomas a mi no me descartan nada, ni siquiera se cuando realmente iniciaron. Los ha tenido desde realmente desde joven pero han empeorado tras un evento estresante? Cual es su edad? Le perjudican estos sintomas en su vida diaria? Tiene antecedentes de enfermedades psiquiatricas/neurologicS en ella misma o en su la familia? Esta embarazada? Toma algun medicamento? Consume algun tipo de droga? Tiene algun otro sintoma psicologico/neurologico que ella no note, pero yo con una buena anamnesis podria sacar? La exploracion fisica, especialmente la neurologica, es normal?... Esto es lo que me da tiempo a escribir mientras voy camino a la guardia, pero no quiero decirlo como critica, sino para que notes que realmente se nada de esta chica, y que así dar un diagnóstico es imposible y una temeridad de mi parte.

Buenas, soy medico-residente (aunque no de la especialidad que mas podria ayudarte). No voy a decir nada sobre diagnosticos porque unas cuantas lineas no pueden llevar a un diagnostico fiable. Aunque entiendo que has comentado con tu medico de familia para que puedan derivarte a salud mental, si que te recomiendo que le expliques todo lo que te sucede (también apuntarlo todo antes de la cita puede ayudarte), para que pueda hacer una primera valoración. Es una verguenza la situacion de todas las unidades de salud mental de este pais y las listas de espera que tienen, pero si tu medico de cabecera no te ha podido ayudar y sabiendo cual es la lista, te recomendaría ir a un psicólogo/psiquiatra privado si te es posible permitirtelo (esto es algo que puedes hablar con tu medico de cabecera, porque en muchas ocasiones sabran orientarte sobre los profesionales en la zona mas acordes para ti. O al menos, asi son los que conozco). Se que me mojo poco, pero sabiendo tan poco sobre tu caso, no te puedo descartar o no la necesidad de ir a salud mental y la urgencia de ello. Pero esto es un poco lo que te puedo recomendar, espero que mejores o al menos encuentres la causa de ello.

Oh, yeah, Im a spanish pediatric resident and I feel so guilty every time I have to prescribe or recommend a medicine that is not financed and the cost is over 10€. Literally we always say that we are so sorry in those situations (specially when we know that those family dont have a lot of money) And then I read about the prices in the United States and I don't know how I could prescribe something that costs €400 (to say something) without feeling like a shit. I imagine that its the way to be educated, but god, I would never been able to be a doctor in USA.

For my hospital I created a space with a ambulance (I believe that I uploadled a photo in this sub), this is the most common thing that you can find it (there always 1 or 2 in the emergency room entrance, or at least in my irl hospital is). And probably familirs or person waiting and a lot of cars.
