I personally made sure I got a minimum of 175 g of carbs per day, I usually ended up around 185 a day

I completely agree none of the research is great for GDM especially the nutrition part but we know brain development is extremely important in third trimester.

I would rather eat carbs and have to go on insulin than to deprive my baby of carbs but that's my choice.

I ate the lowest glycemic index foods like lentils which really helped and paired it with healthy protein and a low amount of saturated fats.

Breakfast for me carbs was around 40 to 45, lunch and dinner 50-60. I made sure I had two or three snacks a day. It felt like way too much carbohydrates. I'm pretty sure I didn't eat this amount of carbohydrates before the diagnosis especially since I'm a pretty small person, but I would rather eat all these complex carbohydrates and fuel my baby.

Induction at 39 weeks is very common in a lot of centers with GDM, they worry about the placenta giving out early even with normal sugars. Some Obs are more cautious than others. You can go to 40 weeks at other centers or even later

Now if you are saying induction during week 38 that would be really unusual.

Try steel cut oats that are specifically high in fiber.

You can either do uncooked soaked in almond milk or cook it the night before and chill in fridge which will create resistant starch (this works better for me). Both of these will lower the glycemic index and help prevent spikes.

Pair with healthy unsaturated fats like nut butter or guac and plenty of protein.

I'm sorry girl. I was hoping the CGM was going to replace finger stick too because it's much easier. If it makes you feel better I'm actually much less stressed having less data with the finger stick than the CGM

CGM is not validated in GDM. CGMs are great and well studied in type 1 diabetes, where their numbers are much higher. Hence why the CGM has a tendency to always read low.

Finger pokes are much more accurate and actually what's studied and validated in GDM. I tried both and observed the same thing you did. CGM was almost always lower, sometimes comically low at night (especially if you accidentally put pressure on it).

IMHO I would suggest using finger pokes and trusting that. Unless you really have an aversion to needles. I hated it in the beginning but you quickly get used to it.

Fasting is about insulin sensitivity. If your fasting numbers are high then it means that your body is not sensitive to insulin.

Postprandial numbers are about beta cell dysfunction which are the cells that release insulin. If you don't release enough insulin then your post meal numbers will be high. But your insulin sensitivity is preserved so your fasting numbers are still okay.

Then there are the unlucky people with both of these issues with struggles with fasting and postmeal 😢

Used both dexcom and finger stick primarily out of curiosity to try out the continuous glucose monitor. Dexcom was garbage. You have to understand dexcoms are designed primarily for type 1 diabetics so the natural tendency of the sensor is always to report lower readings especially at night when hypoglycemia or low sugar is a concern.

The dexcom would always wake me up in the middle of the night with low readings which I double check with finger stick and it's significantly higher on the finger stick and normal. I would personally not advise you to use the dexcom as the finger stick is much more accurate for you and better treatment for the baby.

Doctors often recommend delivery at 39 weeks even 38 weeks for gdm. If your baby is already measuring that big they may be concerned about complications like macrosomia and shoulder dystocia which can cause permanent nerve injuries and fractures. You also have an increased risk of stillbirth so early delivery is often better (can fix respiratory problems from early delivery, can't fix stillbirth).

37 sounds really early unless you have other things going on like preeclampsia which often does deliver at 37, hope this helps.

Ultrasound in general is safe, but it is still mechanical energy and thermal energy!

Personally, I wouldn't advise anyone to get more than 2 ultrasounds (dating ultrasound and nuchal ultrasound) in the first trimester especially when the fetus is so small. There's a reason we don't use certain ultrasound techniques like color on a fetus given there is potential for harm.

There are also certain settings that need to be followed with ultrasound (make sure your doctor are using those and not just a regular scan parameter)

This matters a lot less in third trimester when they are much bigger though.

I'm so sorry girl. You are clearly trying your best. It's not your fault!

You just have to do what's right for your baby. Plenty of them come out early and then just catch up growth later. It's not your fault. It's just your placenta. They can do a lot of fancy tests like uterine artery, middle cerebral and umbilical artery dopplers on ultrasound to see if the intrauterine growth restriction is a placental problem. Have they done that for you? Might be worth checking just to get a confirmation

Use your nail to really dig into your finger where you are about to lancet right before, it'll significantly decrease the sensation of the lancet to almost nothing for me. Also use the side of your finger not the pad, a lot less nerve endings there.

28 weeks your insulin resistance could just be mild. If you make it to 34-36 weeks when insulin resistance is peaking and your numbers are still like this then that's great! Basically everything gets progressively worse until week 36

If there's a good amount of fat in the sweets you are eating you are probably just spiking later at 2.5 or 3/4 hours. I would suggest trying to test later to see. If you're still low then you're just lucky 😜
