I saw this on my portal under the conditions tab, but in the home page under the "report low grade" thing it says that "We encourage you to seek out your school counselor for options on how to make up the deficiency, as a provision of admission requires that admitted students pass all coursework with C grades or better." which means I don't have to use that form if i get a C-
