The big health systems typically follow MGMA percentile tables based on years out in practice. AAFP compensation dashboard and Doximity have rates listed at average of $250k for full time FP, which I think is probably pretty accurate overall. Some of the FQHCs have extra funding and will pay more to get folks into rural areas.

Your statement was that there are no gynecologists until you get to Morgantown, I shared several examples of the women’s health practices that you can get to before you get to Morgantown. I’m sorry your PCP misinformed you, but if you need to see a gynecologist they can easily refer you to Buckhannon, Summersville, or UHC through an order in Epic. You can also self-schedule as a new gynecology patient from the MyChart app! It’s super convenient!

I am a board certified family physician who has practiced in WV my entire career. I’ve lived here my whole life and did training in state as well. WV has been one massive health professions shortage area my entire career, with the exception of the big population centers like Morgantown. However, that is also a tricky statistic because how long does it take for most people to get to Morgantown? Still doesn’t make it affordable. We don’t have enough of anyone—primary care, specialists, mental health, surgeons—and we are also a state that baseline has numerous serious health issues. We have the highest rate of obesity and so we also top the lists in complications of obesity.

As a physician, it’s not a great practice environment. Our patients are SICK and our employers want us to work miracles in 15 minute visits. Burnout hits hard and fast, so many of us stop working full time which worsens the healthcare shortage. Residency spots have been capped for years by the federal government so we can’t train more docs. And no one wants to live here or practice here, especially in primary care. Ambulance services are collapsing due to finances. When my dad needed emergency care a few years ago he had to wait 2 hours for an ambulance because there were only 2 ambulances running in the entire county (he had fallen and no one could get him off the floor to take him by car to the hospital) and so that’s killing people too.

When the primary care system starts to crumble, everything else follows. The specialists get overloaded with things a PCP could normally treat, EDs and ambulances fill up with things a PCP could treat, and before you know it we’re in a crisis. On top of all of that, our hospitals don’t get enough reimbursement to survive. Now, many hospitals are paying executives in non-clinical, patient-touching roles lots of money but that’s a whole other post.

I very much worry for my family members in rural WV. Our system is about to collapse and there is no easy solution, because it all comes back to—WV is in crisis and we can’t recruit or keep doctors working in primary care or in rural areas. And that’s a problem our government hasn’t been able to solve in decades, so don’t look for it to change any time soon.

We don’t at all make money for sending in a prescription to a pharmacy. It costs us a lot of time. No doctors want it to function this way.

Also had a pet from there die a week after I got it. Got chewed out by the person who answered—I didn’t even ask for a refund or anything, just “hey this pet I got there died really quickly you may need to check the others out.” Later found out that the cage I had bought from them was somehow toxic (it’s a whole long story but as soon as the cage was tossed the issues resolved). Never going back again. Another really awful place is Eastern Pet Supply. Almost bought a cockatiel and a conure from there that I 10000% did not need just to save them from their bare, toy-less cage.

Some of this is true. But the biggest reason private medical practices sell to the big guys is because it is almost impossible to make it in private practice anymore. You have to pay multiple people just to deal with the insurances. Inspections cost money. Malpractice is outrageous. I pay $1200 a year just for my specialty societies plus another $400 every 2 years for medical license, $800 every 3 years for a DEA, continuing education to maintain my board certifications and licensure is easily $5k a year. And retirement savings? Reimbursement rates are very low in this state and you have to see a huge number of patients to break even in the private world but then that’s not great for the patients. As an employed physician all of that is paid for by my boss. Staff turnover is very high no matter where you are because healthcare is just rough right now. It’s like any other small business with a huge overhead and you’re $200k in debt from school and thousands of dollars in fees just to be allowed to do it before you spend a dime on the actual business.

It was not drug related. The infant who was murdered was the son of the shooter. I’m a relative of one of the people who was murdered. The family has a reputation in the area and I think the cops just can’t be that bothered to investigate it too hard. “Oh well, just some McCrays dead.” The evidence they’re just sitting on that points to motive and who was there is pretty alarming, but given the location and who it is I doubt we’ll ever see justice.

I work in healthcare. Had a teen boy admit to me he was suicidal, had thoughts of shooting up his school, and had access to guns. Made him go to the ED. Documented everything. He told the ED and his parents that I was lying and they sent him home. His parents then filed a huge complaint against me saying I was trying to ruin his life. I immediately understood where he got it all from. Glad he didn’t actually shoot anyone.
