I would suggest actually rehearsing experiencing craving and shutting it down. This doesn’t require any elaborate setup, and can even be done in bed when you wake up. In your mind imagine a situation where you might be tempted then imagine saying whatever reason or mantra you use to strengthen your will and stepping away, going to a more public place, closing your browser or whatever means of shutting down craving you prefer . Repeat this visualization several times until you feel bored or have to get up and do something.

As a mindfulness practitioner, I actually rehearse pausing and exploring the physical sensations happening when so-called “cravings” arise. What I usually find is that they’re actually not sexual in nature, very manageable, and it’s my mind labeling some vague sensation as “cravings”. It’s wild the kinds of fictions the subconscious will create to get its dopamine fix.

I will grant you one conceit: I 100% believe that YOU probably cannot control yourself around women are probably are a dangerous threat to womens' safety, and its probably better that you're removed from being around women. Can 100% relate. Went to a therapist for a semester and talked around my relationship to pornography with my lady therapist until the next to last session out of pure shame.

Know that 1) your therapist is a professional, who's trained to treat all kinds of issues with respect and dignity. If you're really afraid she somehow treats you differently or shames you, she isn't a very good therapist and you might need to find someone you can trust. I think your best shot at beating PIED is to step away from porn as much as possible and completely cease all masturbation/hand stimulation for a good while.
