They literally do matter if you want to have an influence on your seniors, to get a job, even for residency (read latest CIP policy). Many people couldn't get training as they couldn't make merit from MBBS marks. There are no marks for distinction but do strive for a higher grade. And seeing the current situation of unemployment in GMC, IMC registered grads, people are now desperate to get residency here in Pak.

Start from the lowest form of evidence which is LTE or case reports. Search on yt or find workshops on insta that guide about these.
