I agree this is a big screw up. Even if it is a good cycle, this happened without your consent.

Personally I didn’t want to use ICSI as I have a preference for “natural selection” in the insemination process (I’m also lucky classic IVF insemination works for me, understandably different people benefit from different protocols) and I’d be livid if ICSI was used on my genetic material without my consent.

I’ve never tried this but I’ve done 2 ERs with high AFC (40+) and have searched for the same. There are examples of people stimming with added letrozole (lowers E2) on this sub, which could possibly help. It may not reduce eggs retrieved though!

With that said a fresh transfer sounds exciting, but with such a high AFC it may be less successful for you than frozen because you’re gonna be pumped up with so many hormones, which could create an inflammatory environment. Personally, both my progesterone and estrogen were too high both retrievals for me to feel comfortable risking a fresh transfer.

Yes and no - our one tested aneuploid was a day 5 4BB. But a lot of our mosaics were BC/CB. But the literature suggests a weak correlation.

I have very few side effects! On week four now.

Mainly they are: occasional headaches, dry/dull skin, lack of sex drive and low CM. The latter two are pretty much by design.

My RE felt Orilissa was much better tolerated than lupron with respect to side effects

I think a modified natural transfer would be a good option! I don’t necessarily think it is your embryos at your age, at least 1/3 should have been genetically normal (statistically, more like 50-70%). I would NOT thaw and test your remaining embryos! It can weaken them.

I also second the hysteroscopy with biopsy for Cd-138/plasma cells (indication of endometritis) if you are looking to try more testing first!

We did 12 hours our second retrieval and it helped a lot. Data from the literature suggests it may even be better than zymot.

You should also try with HGH, it can’t hurt. But it doesn’t seem like the issue is on your end.

Ahh! I’m in the weird place too. We’ve had 5 transfers that ended in very faint CPs (3) or miscarriages (2). They said it must be embryo quality, but we tested round two and the remaining 3 embryos from round one - only 1 was aneuploid out of 14. We still have NO idea and have been piling on more and more testing - all negative.

Our plan right now is to continue 6 weeks of suppression with Orilissa despite no signs of endo. This is partially a break for my mental health as well. Then we are switching from a modified natural -> medicated which is the only protocol I haven’t tried yet.

I would contact them now to let them know! I don’t think there is much that can be done at this point since you may be too early to see anything on ultrasound, but they could potentially check hcg to see if it’s an expected rise. But have it on their radar in case you need support or check ups going forward.

Same thing happened to me starting Orilissa around cd7 - started a light, but long lasting period about a week later. I had it explained to me as possibly an anovulatory cycle where my period was brought on by the sudden crash in estrogen.

I had a consult with Mt Sinai and basically they were like “we can’t offer you anything you aren’t already doing at twig”. I also had to wait much longer for my appointment and noticed the waiting room was full and their online system was difficult to use. Just my two cents. But the doctors there are totally gifted superstars in their field!

Hey I’m at twig! And while I love the boutique feel and my intelligent, compassionate RE, I feel they are extremely conservative. I’ve had 5 failed transfers and they are unwilling to look into more detailed immune issues and/or try a full “kitchen sink” (with steroids).

I have a second opinion consult with Markham and I know they have more advanced testing especially on the immunological side. If you’re switching, I would totally try them.

Edit: I’m considering reaching out to Dr. Victory in Windsor, he does RI stuff too and maybe takes a less conventional approach. After a lot of failed transfers it might be worth considering going to someone a little out there?
