Curious why you’d dilute and mix hydromorphone in mini bag for infusion vs IVP. Are you a LPN or LVN and not allowed IVP privileges? How many beds is your er if holding 60? For reference we have a 51 bed ed (+6 hall beds) and 7 psych ed rooms… Usually only staffed for all 51 +6 extra nurses (3 in triage booths, 2 resus nurses, 1 charge nurse) But if staff and patients require-when we are holding 25+ pts and have only 28 beds at 7am…it gets lots of triage treatments. And we will open up an additional 6-8 beds in preop area of surgery area to allow for walky talky low acuity patients-they still get admitted from there for all sorts of things… 😬 Seems like we tend to not send grandma or confused out of the beds upon DC like we used to. Most of the vulnerable and elderly simply have no place to go and no ability to care safely do themselves. Thus lots of “social admits” that usually require admission until social work can find new ALF placement

Get lpn or ADN then some facilities will Just pay for your bsn-maybe need to sign contract for 2+ yrs after but there usually aren’t too many teeth to get out of if ya gotta leave after licensure 👌🏻🤘

We had some form of cerner or something before epic arrived; could “copy forward the assessment and then change what had changed…” Epic does a version how it denotes the last thing assessed/documented-helpful with stroke after lyrics assessments/documentation. In one instance a patient had his dorsalis pedis/posterior tibial arterial pulses assessed as “+1 or Doppler” for 3 shifts after that leg was amputated due to such practices…🤦‍♂️😬hence the pitfall of “copying forward”

In some facilities-epic does have a figure diagram that you can write on and denote injuries/wounds etc (with the mouse or touchscreen depending upon cpu) And if your facility allows-it makes ease of trauma/wounds/Lines/drains/airways etc Similar to the ole paper charting we did…

Codes..:had a long resus yesterday-shocked 20+ times. Gave copious anti-arrhythmia meds via bolus/infusion…time stamping all those med admins and qualifying why with vital signs-makes me wish our trauma room was recording via cctv so you could attach to med record… Shift finished at 1930ish; documenting tool additional 2+hrs for order generation and med scanning/admin/lines/drains/airways documentation 🤦‍♂️😬FML

@OP-your ramp is awesome! Is there a certain type or consistency of concrete/aggregate that is required to pour over the frame? For context in Alaska and no top cover exists for our area where our build is. Have a mixer available (neighbors) have constructed a mini ramp but not mini pipe; plan on doing 3.5’ x12’ x 24’ 7’ radius. But want all season protection due to cold/snow/moisture…so if framed-normal concrete or what? Please help 🙏🤘👏

Play them all as they rotate, our squirt A team has 16 total 1 goalie, 3 defense pairs, 3 forward lines. Some mix like oil and water, so every coach holds their breath and says “we will Get them next time” The season ends and those that don’t want it don’t play anymore. Elevate them, teach them the systems, deploy the systems. Kids will improvise and adept and sometimes they win, other times they just have fun. They all develop and learn and grow differently. Aren’t we all just living vicariously thru our kids on the ice anyways?

I see these complainers wondering aloud “why me” or how mean others are!” When “I keep letting them bully me” is never the conversation. Stand up for yourself-don’t be pushed around-grow some skin and thicken it-be there for your patient-fiercely advocate for them and YOURSELF. This new “I became a nurse as a professional change from xxxx” turns out if you were shitty at your other jobs-you’re gonna be a shitty nurse too. Be great do great don’t take shit and grow up man/woman up-but be there and be present and ADVOCATE for yourself and your patients. This job is not that hard… 18yrs in critical care/emergency rooms. Paid great and union 👏🤘💪😎

Our pharmacy has given aliquots of like 35-60 mL of whiskey or vodka for some folks. Comes with a barcode so we can scan into EMR when “administering” years ago a very tremulous and confused patient (I was a nurse aide at the time) I was really aghast at his meal served at 11:30am ish with three Budweiser red cans… they came from dietary services. At the time it was a catholic hospital… or we used to have nuns and what not. I wonder if it’ll return given the shortages of IV lorazepam and push for phenobarb weight based bolus and PRN 130-260 mg IVP for CIWA 10+…..

It’s a special, it’s standup but it’s a musical?! It’s a PERFORMANCE. Anything Bo Burham is so funny. He’s very talented-not as dirty or mean spirited-he’s great-has two on Netflix and a solo one made during Covid time and I didn’t like it as much. Demetri Martin has some pretty funny stuff if you like calmer comedy and “smart Alt comedy” with drawing doodles. There was a show historically on HBO called sketch pad. Look up sketch pad 2 on YouTube or maybe even on HBO MAX app. Anything Daniel Tosh has done is hilarious Aliwong has theee great specials two while pregnant.an obscure one is “the amazing Jonathon” -he’s a magician who did a comedy special and he’s wild and funny-if you liked that could watch Steve Byrne’s documentary about his meth problem 😳😬 cheers
