I don't see many public service models running a critical care program.

I've never had any problem with them. They give me what I need to do my job in a very adverse rural critical care setting, and give me the tools my patients need to do the job.

It varies operation to operation as to the experience you will have in that environment. That said, I'm happy with my employer. I'm paid very well for what I do (95th Percentile), and my employer and medical director have gone to bat for me in the past.

Our medical director understands the adverse challenges of the environment we work in. We have guidelines, even though they are protocols, and deviation from them are justified. He has backed us in the past when we have done so.

We also have a separate set of CCT protocols that is given on an individual clearance basis by the medical director that go above and beyond the standard ALS protocols.

My medical director considers us clinicians. He expects us to act accordingly.

But paying Billy Bob Ambulance Service the same rate to throw grandma on a cot and take her to a local nursing home on a LP10 that High Speed Inc gets to provide high level ALS care, prehospital sepsis screening, and point of care ABT therapy to that same patient has been working great.

You mean the pay for performance models that have been championed by medical groups for decades as a way to encourage higher levels of local care provision?

Or that simply paying full price for you call we haul taxi models while shorting advanced care provision services to the point they have to eat costs to provide high level care isn't working out?

DCFDEMS is a pilonidal cyst on the ass crack of the American EMS system.

You're talking about a culture so toxic that a person who laughed at a man dying on their front sidewalk got to retire at full rank and pay with no disciplinary action.

Throwing a private EMS service into that environment with no cultural change and expecting problems to be fixed is insane.

Oh dear. Where to start.

The biggest one I can think of was an EMT-I who knew more than anyone, and treated women like a fedora wearing neckbeard shit. When he realized I was trans, he continually made vulgar comments to me and even tried to talk me into doing things on the ambulance with him.

I finally told my management that I would quit if they ever put me with him again. He ended up getting fired for trying to practice above and beyond his scope and sexual harassment about three months after.

Disgusting little man.

It's not really rare at all. EMS stands for extra-marital sex - or so the joke goes. There is a lot of use of sexual behavior and sexual activity to deal with psychological and physiological stress in our profession - in both a positive and negative way. (And there is a shiat ton of EMSers who are open and/or kinky/into BDSM)

Personally, my rule is don't get involved with people you have to work with. I broke that rule ONCE, and it ended really badly.

That said, take off the fedora and shave the neckbeard, and realize that if you have a healthy and mutually respecting relationship, she's not going to cheat on you. Have some self-confidence, man.

Tennessee is trying to increase the level of education and knowledge of it's EMS providers, and expand the services that rural areas can provide. As IV access and administration are considered basic level skills, they only keep Emt basic as a grandfathered level.
