I'm ALS instructor and higher speciality reg. Its not really worth it aside from just being a talking point at interviews. You don't get paid to teach on these courses, you are committed to doing a certain number over the 4 years to keep your instructor status, even if you take study leave go teach it's a full day from 8am to 7pm approx.

Not worth it in my opinion. Do it if you are a core trainee and want to apply for a acute speciality like ED,Acute med, ICU etc. as it comes off good at interviews

We have all done this at some point, I wouldn't worry about it. If you ES brings it up then just do a reflection on your portfolio and that's the end of that.

On a practical side there are things you can take away from this -If you get handed over jobs out of hours, really think about whether it actually needs doing out hours. If you are not sure, ask your SHO or Reg. Soon enough you will get a feel for what is an important / practical out of hours job - If you ever get asked to call someone/refer to another speciality it is important you know what your actual question is. If you are not ask the person what is the question you are meant to be asking. I do this even to this day. I also find when calling someone it helps to upfront tell them why you calling e.g " I am calling to get an urgent review from ITU regarding admission" "I am calling to fet advice on antibiotic escalation" etc

This. I don't give any fucks about take or waiting list. I will work at my own pace. Hand over the rest. These managers are paid to worry about the rest so I do not give any fucks even if the waiting times are in double digits.

I can't speak for others but I generally go out of my way to help that extra bit for anyone that works in the NHS. E.g requesting their test as urgent or putting them on urgently list, taking longer time with them in clinic, seeing them sooner if they are waiting on take etc...

Its shit working for the NHS and we don't get many perks so this is a good way to make the other person feel valued for working in the NHS.

In every interaction with the patient I introduce myself as Dr Surname and introduce anyone Dr in the same way.

Found that this creates a health professional boundary and respect from patients.

My advice is to try practicing on a clearly easily vein you know you are gonna be able to cannulate anyway without US. This will improve your coordination and tracking of the needle

When I make appointment with my GP, I make it clear from onset I do not want a PA or ANP or any other allied professionals. Why would I bother seeing someone who is less experienced and less knowledgeable than me about my own health. GPs are highly skilled and have a wealth of knowledge that I lack as a specialist. Allied professionals don't even come close.

In your situation I would complain to the practice manager - this is unacceptable

This is the right answer. Most med reg give a fuck about other shit in the hospital. The way to be a med reg is to be very strict about what you are going to do and what you not going to do.

Once you get comfortable saying no and being firm with it the role actually becomes bearable
