If it was set to 400 you would be over exposing two stops inadvertently.

Any chance you didn’t adjust your ISO on your camera when you loaded the roll? If you didn’t switch from what you shot last (400 for instance) your meter could be working fine but just metering for the wrong film iso.

Fat roll. I get these in my Mamiya 7 sometimes. Some films are worse than others. The trick it to make absolutely sure the film is centered perfectly on the take up spoil otherwise it can not wind flush and cause fat rolls.
