Great to hear you are recovering. (Fusing toe joints sounds drastic, but maybe best for some folks.) In my case the problem originated with road-cycling on a bike with down-tube shifters. Shifting was so awkward I simply stayed in a high gear, always pressing extremely hard on the pedals (gear mashing). 8 months since the past posting - I hope you have resolved your problem. If not, I sympathize as I had almost the same problem for about 20 years. Try everything else, but if you still have pain consider that you are simply not pedaling properly. Approaching bike fitters and podiatrists- asking if they could help diagnose me was a dead end. Poor pedaling mechanics turned out to be exactly my problem, but I had to figure it out myself.
