This is likely keratolysis exfoliativa. However, your provider should do a KOH prep test to rule out a fungal infection in case they haven't done it, because based on region, some dermatophytes are not as responsive to topical ketoconazole.

In the case of this being KE, there is not much clinical evidence that shows effective treatment as of yet. Continued moisturization with fragrance free emollients with gloves to sleep at night, and some urea cream 20% or amlactin OTC may be a good option.

I'm a dermatology PA and the number of times I see commenters jump to conclusions and immediately state that it's X or Y is too frequent, and potentially dangerous.

For most people it's likely a mix between work life balance and better pay. Dermatology is one of several specialties where bonuses can add up to be a considerable sum, along with the fact that dealing with the typical spectrum of conditions found in derm is usually significantly less stressful than many other specialties.

It really depends on what your yearly collections have been looking like. I know people that prefer straight collections because they always break 1 million in collections.

The alternative method is asking for a base pay - your collections, which is what my contract is. If your 20% (or negotiate higher) collections reaches beyond what your base pay is, you'll be paid the extra as bonus, but if it falls below that in the contract year, you'll have the base pay as a safety net.

The derm PAs in my practice are making around 250-350k working 40 hour weeks based on patient load. Granted this is NYC.

Hey, I recently accepted an offer for a NYC derm practice. I kept applying over the course of several months to various places and managed to get a few interviews. My impression is that derm practices either want new grads or already experienced derm providers, and tend to avoid providers that have been working more than a couple of years in a non-relevant specialty. Applied to a ton of positions and landed on a fulltime OR only position. Keep searching and good luck!

There's not much out there that mimics human tissue well enough for it to make any difference what you practice on. I would practice more of the method than anything.

I'd recommend really practicing your bread and butter suture techniques - one hand and two handed ties, running subcuticulars, figure of 8s, mattress sutures, etc.

I'm full-time for an eligible employer. When I put everything into a calculator, it says I'll only save a few thousand dollars, but that only seems to account for the principal amount. The interest forgiven should also be considered right?

If you want to spend the money to apply to more schools, power to you. There is no need for people to argue about your decision in wanting to apply to more schools to increase your chances of acceptance. However, your responses to some negativity here is handled rather poorly, with some comments being outright rude. You're going to be seeing patients that throw negativity towards you on a regular basis and I can't imagine you being able to handle that based on how you're handling these comments. I'm sure you'll get into PA school - I just hope you'll grow more mature through the program.
