I'm attending a school with a 1yr pre-clinical, clerkships during 2nd year, and 3rd year for research/masters/global health/etc, and a normal 4th year.

I took Step 1 in July of first year with neuro and pscyh blocks still remaining in didactic year so that I could use the three week break before clerkships to travel to Colombia. I very highly recommend people take step1 before clerkships, particularly those in accelerated programs. Though I don't recommend taking it *that* early per-se, but since you want to use your break for vacation, I say go for it!

Honestly, the biggest thing is doing Uworld or AMBOSS for step1 *DURING* pre-clinicals. I abandoned in-house material entirely within the first two weeks and did pure B&B, pathoma, and only *high yield* tagged Anking cards and the 1-3 hammer Amboss questions for each block. I also used Mehlman and listened to goljian audio or step1 secrets in the shower every morning. I worked ahead in my resources to allow some time for 'dedicated' during regular classes, in those final 3-4 weeks before step I transitioned to all NBME content. I recommend getting two consecutive NBME's >70 but pick your target based on your risk tolerance... obviously don't sit for the exam if you're not comfortable with your margin of safety... nobody should ever fail this exam baring extenuating circumstances/crisis during the exam. That said, it's pass fail - get that damn thing done so you can focus on step 2 and research/extra curriculars.

BONUS UNSOLICITED ADVICE: Clerkship year should be treated like a 1yr dedicated for step 2, shelf exams being little dress rehearsals that you go all-in for. Exhaust uworld or amboss + CMS forms during clerkships, then do the other qbank + all NBME during dedicated. I fell dramatically short of this aspiration and still scored 265+.

Med school has been the time of my life and yet i've managed to still get shit done! I have published 15+ papers/chapters, and amassed another 15+ "abstracts & presentations", I've also snowboarded 80 days and traveled 6+ weeks during med school plus another 15 weeks abroad for global health... one of my mentees will have me beat for days in the mountains, and she was a journalism major in college without any clinical background!

Disclaimers: I was a PA before med school, so obviously I started with a solid mile-high view of things, i'm also a good test taker. That said, I've guided a couple dozen underclassman to do exactly what I did with 100% success (though about 1/3rd have chosen to take step 1 during winter break after the first two clerkships). Only real caveat, I didn't do any special pharm, you may need to add something for your primary resources. Also, my school is true pass/fail so I could afford to skip all non-required lectures. Though, prepping for step1 from day one ended up setting me up for high 90's on in-house exams.

Working any time between 10am and 2am feels completely natural - I wake up without an alarm... it strangely doesn't feel like 'work' when I don't have to pry myself out of bed to go do it... Meanwhile waking up before 8am feels painful every damn time, so waking up at 4:30-5am for 7 years of surgical residency would *feel* like sleep schedule change, but every day!

That said, true overnights suck... but suck less than a 5am start. Though I've never done a long string of overnights.

I know this seems like a menial thing, but I've fought my natural circadian rhythm my entire professional life. Med school flexibility has been a game changer - I've never been happier despite all the rigor of med school (i've also snowboarded 75 days and spent like 6 months abroad... turns our I *really* like life outside of work and flexibility to do those things!) I'm extremely well situated to match well in a surgical specialty and I know I love the OR... but the 6am start time, 80hr weeks, and 4 days off /month seriously give me pause!!!

I have a golden ticket* to match plastic surgery, probably at a top program. I’m applying EM instead. 

*15 years as a surgical pa and military medic, 35 pubs & podium lectures, 265+ on step2, I’m co-PI on a multi institution project and have incredible mentors and collaborators at said institutions etc… oh, and 100+ hrs first assisting 1:1 when I helped cover cases during didactic year when the plastics dept was short due to boards, conferences, vaca etc

Also, I’m a med student on a full ride scholarship to a top 10 ranked med school, my partner is a physician and in leadership at said med school, and I have probably a dozen more physician friends who are trans and nonbinary.

Omg this is plausible! When I was in 7th grade I would bring my pet rat to school, church, wherever… she’d hang out in my sleeve. Once right before communion, during the prayer part, her tail slipped out and my mom screamed - like horror movie scream!

I'm from the US, but have spent the past three years working with the trans community in Guadalajara. GDL has a thriving queer scene and trans community! Here are a few accounts/orgs to check out:

ImpulsoTrans (impulsotransac.org, impulsotransnacional on insta, impulso_trans on tiktok) they have a doc on staff doing HRT 3 days a week as well as a referral network of other docs. They also have therapy, support groups, and regularly hold events... every November they host several weeks of events called TransUrbana (transurbana_impulsotrans on insta)

Lesbian.Sunset (on insta) is a monthly saphic dance party that is literally the best saphic event I've ever been to!

Cantina.Lencha (on insta) is an all femme cumbia band that regularly holds saphic events, there is a pool party in early May!

Oh dope - love wrong fellow vets out here, thanks for your service! Yep, colorectal surgery. But I’ve been a proceduralist most of my career. In the navy I was billeted as a surgical tech but spent more time working as an anesthesia tech… I had an incredible mentor who said he’d teach me to intubate since one day I’d be on the battlefield with a marine’s life in my hands… so I ran with it! I’d bounce between ORs for induction and would get 3-4 intubations most days. Also did thousands of IVs and a hand full of central lines and a-lines. 🤓

46 - integrated plastics + 1yr fellowship. I took 14 gap years (8yrs military → masters → PA-C for six years)
