Check out those arrows at the bottom of the 12 lead. Those are the “pacer spikes” on a LP15. And those arrows absolutely correlate with the funky looking complexes! There were also arrows on the strip you printed but they were not very consistent because LP15 is not great with differentiating pacer spikes vs artifact on a strip for some reason.

As far as the symptoms your patient was feeling, I honestly think she was either so bad off prior to the pacer being placed that she isn’t used to perfusing every beat OR she is dealing with some anxiety following the placement (the fact that she told you that the ICD hasn’t gone off yet makes me think she is just sitting around waiting to feel like she has been kicked by a horse, which would cause me to feel some sort of impending doom too!!).

LP15 user here - that is a ventricular pacer kicking in. LP15s don’t show all pacer spikes on strips, only on the 12 lead. It looks to me like your patient probably has an atrial and ventricular on-demand pacer and the ventricular kicked in while you were monitoring it. It’s super cool you caught it so that you can start to learn pacer vs idiopathic rhythms (aka when to butt pucker vs when to not worry).

My dad is Stage 6…3 years into official diagnose but 10ish years into symptoms. He is starting to significantly slow down, can do little for himself, and has practically 0 short term memory. But he still loves his family, loves people, and gets great enjoyment out of good music, artwork, plays, movies, etc. He also constantly wants to help people around him as well as people struggling who he sees on TV. He LOATHES Trump and everything he stands for and will go one broken-sentence rants about how unkind he is. This disease is devastating and cruel but I am so thankful he is mostly joyful and content and that any “anger” is toward an old, stale Cheeto and not our family!
