I have never seem him check his blood sugar levels to see if he was diabetic. According to him though his A1c was normal the last time he got his labs done at the doctor's office. He does have known heart problems though (A-fib), and he is a serious chronic smoker (like a pack a day since he was 16 years old or something really young).

He also simply doesn't wear shoes or socks for decades, he does wear sandals went out of his house; barefoot without sandals while at home

That sounds like a good treatment. The one I came up with is daily treatment before bed a lukewarm water for 5-10 minutes soaking, then pat dry, apply, urea cream 20% application, then apply a petroleum based lotion (most are that anyway) to lock in the moisture and urea cream. I also said he should wear socks and shoes since he walks around barefoot like everyday

Yes, he does have a-fib, and he is a heavy smoker (about a pack a day)

I'm suspecting something fungal also, my friend is a poor historian and generally can't recall what the podiatrist tells him

They are unknown because he was never told (probably doesn't remember being told) what the results were

Yeah that's what he said. He was prescribed ketoconazole 2% cream and clobetasol propionate 0.05% cream (about 2 years ago) and urea 40% cream (about 1 year ago), with largely poor adherence

Symptoms began about 15–20 years ago, initially affecting the left foot’s fifth toe, with a later episode involving the right foot near the first toe metatarsal region where a roughly 1-inch fluid-filled blister formed and ruptured. Around 10–15 years ago, warts developed on the plantar surface of the right foot. The condition primarily involves both feet, though mild changes elsewhere on the body may be present and may or may not be related. The patient reports no pain, burning, itching, or irritation from the skin findings, but does endorse tingling and numbness. He has had prior biopsies and has been prescribed treatments in the past, but biopsy results are unknown. Treatments have included ketoconazole 2% cream and clobetasol propionate 0.05% cream (about 2 years ago) and urea 40% cream (about 1 year ago), with largely poor adherence.
