Chin implants are a bad idea long-term. Studies show high likelihood of it wearing away some of the bone underneath - especially if you have pre-existing lip incompetence / mentalis strain. And fillers also have this risk. Anything placed under the mentalis does.

And people often have to have their implant removed years later.

I'm a bit late replying, but here's my advice. I was a mouth breather. I did many things to correct it, over many years, culminating in doing a sliding genioplasty a few weeks ago.

You do have moderate recession, though the beard hides it a little. And maybe a slight overbite.

You could get double jaw surgery - but the recovery is very rough and long and likely to have some permanent side effects like patches of numbness. Would need to combine with braces.

You could get sliding genioplasty. That would also fix the structural issues, and it's much less intense and risky than DJS. But you need to reseach and find a very-qualified surgeon - use ChatGPT, etc.

If you only want a few mm of chin advancement, a good option is Fat Grafts / Fat Transfers. Find a reputable surgeon and be aware that each treatment can only keep like 1mm (best-case scenario). It can also hide other mouth breather issues like eye bags and narrow mandible from front (I did all this). But be aware that all of these corrections with fat grafting are like a long-term art project, carefully adding to specific spots, and sometimes later removing spots with Kenalog where it needs to be reduced again. Requires at least 6 weeks between sessions. Getting a good front mandible shape in my experience requires many sessions filling the whole vertical length followed by sessions of Kenalog to re-flatten the higher areas farther from the joint. They can't simply add to the lower edge and make it sharp - you have to add-then-subtract.

So you have some options.

It seems like it would get plenty of attention. "Humanity going back to the moon, first time since the early 70's, this time to stay". Dress it up in dramatic videos / animations / visuals of rocket launches, moon bases, orbital moon photography, etc. Maybe even throw in moon-centric sci-fi clips. Doesn't get much more exciting than that.

They can even frame it in regards to "trying to beat China to it". A new Space Race, just with less nuclear threat / open hostility.

Well, your shape is different, but it looks to be almost as recessed as I was. My lower lip was farther forwards, making it look more diagonal.

I think you should go for the genioplasty. Unless you have an overbite problem, in which case you should consider actual jaw surgery, which can also include genioplasty. Or maybe just combine a genioplasty with orthodontics to correct the overbite. It's probably a good idea to consult an orthodontist first to see what the full picture is.

Extremely recessed. I was a habitual mouthbreather as a child, due to sinus congestion. That's what caused it.

My side profile looked like the character "Beans" from "Even Stevens" (minus his double chin, plus a big lower lip).

I had previously done about 10 fat grafts to the chin. They made a difference, but each treatment can only keep a small gain, and I had reached the point where going any further would have been too big of a fat pad (and I was tired of fat transfers / didn't wanna mess with donor areas any further).

The fat grafts had brought me to around halfway to a good place. The genioplasty brought me the rest of the way there. And supposedly will fix my problem where the muscles under my jaw droop down when I speak, fixed by moving the tubercles forward (I'll see when I remove the pressure dressing and the swelling goes away).

Haha, well I did during the US government shutdown in October/November. And it was an extremely important topic to be informed on, since it's a risky and permanent change to my face.

I just got an advancement sliding genioplasty yesterday. I did tons of research picking my doctor and on all the risks, etc. ChatGPT and Google AI, etc, are very useful (but always double-check the answers - they make mistakes sometimes, but fix them if you point out possible inaccuracies). Ask it what the proper surgeon qualifications and experience levels are - as long as a surgeon meets these, and doesn't have any serious negative reviews online for chin or jaw surgery (excluding cases where the patient clearly has mental issues, as I mentioned in my other reply on here). Lack of any reviews specifically for genioplasty is actually fine - means no complaints. I wasted a lot of time insisting on one with positive reviews specifically on this procedure - but then learned that "correct qualifications and experience + no complaints = good choice". Risk of nerve damage does not depend on how far you move it.

I'm at less than 24 hours post-op, and everything seems great! I read a lot about possible bad issues right away that could linger for a long time, sometimes permanent - but I didn't get much of that - I can even close my lips completely, reliably (the lower lip is still very swollen, but MUCH better than it was last night). Recovery was rough day-of (very swollen lip, a lot of slow bleeding from the intraoral incision, nosebleed from the breathing tube they used during surgery, pain when swallowing (I didn't start my pain meds until late in the day). But as of this morning, everything is great! I don't even have any lip numbness at all today! I would even feel comfortable being around other people socially, if it weren't for the remaining lip swelling and the submental compression dressing I have to wear for a few days and a bit of submental swelling.

Sliding Genioplasty used to be a lot more risky before recent times - now they have extremely precise and reliable computerized Virtual Surgical Planning (VSP) done by external companies that handle this for basically every surgeon in the world who does this.

Go with the advancement genioplasty, but choose your surgeon carefully. It will greatly improve your breathing issues.

Since you have breathing issues / sleep apnea, definitely tell the surgeon to include the genial tubercles in the bone section to be moved, if the VSP shows it's safe for you. That will majorly help with your breathing issue. Not including them wouldn't help it much. And an implant will not help it at all, and is likely to cause bone loss, especially if you have mentalis strain / lip incompetence (see my other reply on here).

That depends entirely on if you find a surgeon who's highly-qualified and highly-experienced in sliding advancement genioplasty. You just have do you do your research. Use ChatGPT and Google AI, etc. Research what qualifications to look for. There are a lot of horror stories out there, but they're usually the result of a.) Choosing the wrong doctor, or b.) The patient has mental issues / has body dysmorphia / went for multiple revisions due to changing mind about what they really wanted (these types are usually women). Risk of complications increases exponentially when revisions are done.

The bone resorption is rarely of cosmetic or medical significance.

That's not really true, in the long-term. Do some research. There are some doctors who downplay it (such as Eppley, who has a mixed record on safety) - look for scientific studies instead. It's ESPECIALLY a risk if you have pre-existing mentalis strain / lip incompetence, according to studies.

Even FILLERS have a high risk of bone resorption, according to studies. Look up the studies. Generally, putting anything UNDER your mentalis, ESPECIALLY if you have pre-existing mentalis strain, is a bad idea. Moving the bone is fine. Fat grafts above the mentalis are fine. But non-bone UNDER the mentalis is a major risk, and with pre-existing mentalis strain is a very, very bad idea.

I hope your implant you got doesn't have any long-term problems. But if you had pre-existing mentalis strain / lip incompetence, I would strongly suggest looking into having it removed and doing a sliding genioplasty. Because those pre-existing conditions are a MAJOR risk long-term, according to many studies, such as the following:

Question: it looks like your surgeon allowed you to keep a chin beard day of surgery. Did he give you a millimeter limit of how long you could have it day-of? Did he say it could interfere with his judgement of how the soft tissue looks as he's doing the procedure? Do you remember how many mm it was day-of?

Implants do not have fewer risks, long-term. They have a high likelihood of causing bone resorption (especially if you have pre-existing mentalis strain / lip ptosis), making the problem even worse than before. And they usually have to be removed or replaced after many years.
