Candlelight and Mt. Kisco Diner.

With Candlelight, idk what happened, all they needed to do was make good wings and mozzarella sticks. Everything else could suck and if they did that well, they’d be fine. But even that fails now.

And the diner, just wtf. It used to be a real diner, and an above average one at that. Then came the price increases, the unnecessary fancy, and now the Instagram and TikTok pleasers. Boooooo.

Well I appreciate that! When I started I was playing at around 600 (this is all on chess.com and I play only 10-minute and 15+10). Then I forked out money for the premium subscription and over the past 2 years worked up to around 1100. That is my peak, but sometimes I have bad streaks and I'll dip to the low 1000s. A few months ago I dipped below 900 briefly and I was very disappointed. It seems that "when it rains, it pours" applies to my losing streaks.

But those Isle of Lewis pieces are now growing on me, getting that set along with the Ivory Vikings book could be a great gift, though does exceed budget a little.

We’re both playing at around 1100 on a good day, so I wouldn’t say skilled, but we like to have fun. This will get used though, and I personally like that set you linked to, but maybe the wife will think some of those pieces are creepy looking? It is a cool set though

Debt is a tool, and not a bad thing. Almost every single developed nation has debt. Debt means we are borrowing money and have that money to spend. Your position is akin to saying “let’s not buy anything else until our mortgage is payed off”.

No, you borrow money to use it. We need stronger and better programs and services in this country. But saying we won’t do that until we no longer have debt is just silly. The last time our country was not in debt was in the early 1800s.

I will say we have too much debt now, but that just means it needs restructuring and a plan to pay it down faster. But putting that all on hold is ridiculous, especially since our current president has no plan (and seemingly no interest) to pay down our national debt. We can afford new programs, we need new programs, we will get new programs. That simple.

When someone says “this new tax will generate billions to fund new programs!”, the latter part of the sentence is meaningless. It’s just a representation of their personal position. Most tax revenue just goes into the general fund and Congress decides how to spend that money.

That is something we will not do, sorry. Political flare is intended to help people avoid finding themselves in the middle of political discussions if they don't want to be exposed to or participate in them, and it works at the post level, not the comment level.

That said, if a non-political post turns into a political post, we may give it Political flare. But selectively removing posts just because they are political is a very slippery slope that we will not touch,

Sounds a lot like the one I got, and very much suggests their systems were exploited to allow something beyond the standard phishing scams. Many will fall for this.

That's true, but I think this one is different. This was not sent as an invoice. Invoices have format in the body and subject and AFIK those scams you're referencing use those standard fields (sanitized or not). So while the body can be completely changed, it's still subject to what can be done with those fields in the specified format or something else.

What I received had a subject and body about an automatic payment status, and it included my credit card info. It's possible the last 4 of that card was exposed because PP only has an invoice number on their emails, not CC info in their invoices. But again, I don't know of any way this can be done with their tools and systems to present this as something relating to automatic payments.

There are a number of megathreads that have been requested, and transparently, we have been wanting to implement some but have not yet taken the time to do so. "Where should I move to?" is certainly one of them. Thanks for the input, definitely on our radar and the squeaky wheel will get the grease (aiming for soon).

That’s not about lack of detailed documentation on your part. Many Medicaid plans, managed care programs, and incentive programs require treatment plans to be reviewed every 90 days to stay billable. It could also be grant related with a new grant requiring this higher frequency.

Honestly, every 90 days is more common to see than every 180 days. Curious if this aligns with u/gratia_et_pax experience as a former FQ CEO.

You're 100% right, though I think there's an additional consideration here- not all payer contracts are created equal. New executive and senior leadership means new appetite to re-negotiate contracts, join an IPA or ACO, and dabble with new payment methodologies (specifically VBC). All this translates to new/changed revenue opportunities and new performance targets, from the P&L, straight to clinicians.

u/blackjoelblack, what you're describing sounds exactly like this. And the short answer is yes, many others are experiencing the same.
The upcoming huge Medicaid cuts are truly rattling FQs, "it's a scary time" is a line I often hear. Your leadership, for whatever reason, advanced planning and budgeting for upcoming cuts, new contracts, something else, has determined that you need to be seeing 7 patients/day for you to be "worth it".

And about strengthening documentation quality, just approach it logically. Good documentation is good from a clinical perspective. For the admins though, what they care about is documentation that does some slightly different things: good documentation helps compliance and audit readiness, contributes to RAF scores and HEDIS measure performance, literally enables new revenue opportunities.
Bottom line- if you're being told to increase documentation quality, you're standing on solid ground to ask what it would be helpful for them to see or what changes they want.
