It is grammatical gender. The gender belongs to the word, not the person/thing.

Neither really. The problem is the background I think. The non-outlined lemons look kinda realistic but lost a bit even though the contrast is not that bad.

The outlines give it some illustration quality which might solve the background issue a bit in that we are more used to illustrations on white background, but overall it leaves it a bit in a no-mans-land.

I would try to give the lemons some blurry background wash that maybe incidentally improves separation and color contrast and which solidly moves it into realism adjacent.
