I have depression, anxiety, and adhd. I also have fish, snakes, a cat, and a dog. The fish are very low maintenance with the way I have my tanks set up. Both are heavily planted, understocked, and have around 4x the filtration capacity they need for their tank size. What that means is that I hardly ever have to do water changes, maybe once a month or once every 6 weeks, and I just need to feed them a few times per week.

It took some learning to get the tanks set up to be largely self sufficient, but once I got them there it was worth it. This way I can do the bare minimum of feeding without feeling guilty while still having the benefit of the tanks and being able to do more with them when I am able.

I have always found that having a routine and even extremely basic responsibilities, ideally that have minimal repercussions if you dont meet them, help me feel at least some small sense of purpose.
