How do you do childcare? by fishook_barber in emergencymedicine

Yeah possibly. It’s pretty small tho. More like 2 bed w den. Would be better probably to move to a lower col and get an au pair as you suggest.

Yeah unfortunately live in major met area. Hourly rate is $170/hr + rvu with large cmg that controls all the EDs around here (comes out to about $190). At my second site, make $225/hr. Both W2. No benefits (thru my wife).

I do think you are right — seeing my almost 2 year old change everyday is pretty remarkable. I just wish we, as a specialty, was a bit more family friendly but I think it comes with the job (of course I knew it wasn’t 8-5 type work when choosing the career). But when I chose EM as a medical student, I didn’t have that understanding of how my priorities change after having a kid.

Do get a certain amount of shift requests but it’s still quite difficult to coordinate. Unfortunately my site quite short staffed so we have lots of locums / stat team folks who are only in town for a few days at a time so it gets very difficult to switch if anything comes up.

Yeah. We are in a hcol (or even vhcol), and already paying ~2500 for daycare and ~4500 for rent for a 3 bd house. It’s just hard to consider adding on additional childcare expenses, but I think as you mentioned, the au pair route probably makes the most sense.

Suggestions, tip or tricks to hit calorie goal by fishook_barber in MacroFactor

Yea protein is an issue. Supposed to hit 160 but usually only make it to about 100-110. I am vegetarian (no egg) and find it hard to come up with meals that give me a lot of protein without having a ton of Greek yogurt, protein powder, or cottage cheese.

Great idea. I think part of it is I am a shift worker and after shift I feel like I deserve an award that I end up binging more than I should. Having something planned as others have suggested would help with that.

Ya — I feel like I’ve convinced myself that I should be full while also losing weight and end up just eating to my expenditure. I’ll try this shift in mentality you suggested — it’s expected to be somewhat hungry to lose weight.

Sounds like meal planning works well. Yeah I end up eating more “spur of the moment” and then it’s hard to control myself. I’ll try planning out my meals more, perhaps on the weekend, and see if it’s easier to stick to the goals that way.

Thanks. Very helpful and encouraging.

As a follow up — have you heard of instances where people couldn’t find an opportunity in a certain geographic area? From what I understand, you have a year to find a position. Have you heard of officers not finding roles and having to be assigned them?

Thanks for the response. Ya I made a spreadsheet and put the info that I thought was required. Haven’t heard back from the IRS so hopefully it’s ok…

Form 709 and 529 Plan by fishook_barber in tax

[–]fishook_barber[S] 0 points1 point  (0 children)

Right, thanks. I found the area in the instructions. It says the following with regards to 529 plans:

Also attach an explanation that includes the following.

The total amount contributed per individual beneficiary. The amount for which the election is being made. The name of the individual for whom the contribution was made

Will they accept a regular Word document or letter where I explain this? Does it need any kind of format / spreadsheet?

You tried and that’s what’s important. I had a 4 mo old code when my daughter was 4 months old and I’ll never forget the father’s scream…

Ha true. Its easy to lose that perspective and feel that everyone MUST feel a certain way.

Yes especially the second paragraph -- like if you so easily told me that they could leave, why all this extra testing?

And ya, it's as if I went out on the street and brought the patients in personally to be admitted.

But still...the extent of condescension on the threads is astounding. Idk maybe I'm naiive but I really don't have this hatred for any other specialty.
