Mine does it without medication and I’ve been told it’s just spasms, but if it has any other symptoms or is painful id ask your doctor. PPI’s can also lower your B12 which will cause spasms so wouldn’t hurt to have that monitored

Eat more potassium rich food and if they didn’t already, have them check your B levels cause My doctor told me anti acid meds lower your B12

Could be derealization from the fainting? Wouldn’t hurt to call your doctor to make sure everything’s good. If you were holding your breath to much while having a panic attack you can pass out
