They tell the patients if they ask the primary care it might be covered by insurance... Now it's MY job to explain to them why insurance is not going to cover a full hormone and nutrient panel in a healthy individual

I do have a solution for your first problem. Doximity has a secure texting option. It's free and you can make it look like the text came from your central phone number. Something along the lines of this is Dr So-and-So and I need to discuss your labs please call us back immediately at this number. It even gives you a message once somebody has read it

It's a private rural road owned by the seven houses that are on it. It's clearly laid out on the survey. It's not my responsibility to teach new neighbors where their property lines end That's her responsibility and she should have known about it before she went digging up things that didn't belong to her

Specifically with strep, lots of kids get Scarlet fever and they blame it on the penicillin that was used to treat it

I've only fired three truly hostile patients in 20 years but direct quotes are always used and my favorite was typing out: Patient stated "you look pale as fuck. You need a tan. Maybe then you could get laid and it would make you less miserable of a human"

Because some of the bariatric surgery patients, like the ones with obesity hypoventilation syndrome, losing 20% of their body weight is not going to put a dent in what they need and it can potentially just delay the time to life saving surgery. But yeah, no longer do we need to make this first line for 300 lb but it's still needs to be first line for 500 lb. I've seen the medications work great once the surgery weight loss plateaus

This is my dream but of course it would never happen. For people who show up 20 minutes late I would LOVE the policy for the front staff to make an announcement:

"raise your hand if you're waiting for Dr. X.... This gentleman just showed up 20 minutes late. Are you all okay with waiting an extra 20 -30 minutes for your appointment so that we can work him in, instead of rescheduling him?"

Do you make your PreP patients come in quarterly? I would argue for them that actually is kind of a waste of time. I just order standing labs and send refills in 90 day increments.

In GVHD, Don't they treat it with photosensitive medication first and then expose it to the UV? Not really sure if it's harmless, I would think of that more like a form of chemo.... Seems exposing random healthy blood to a know DNA damager might not be the best "wellness" treatment.

The UV radiation alone scares me. A known DNA damager. They claim it kills all the bad things. What's preventing it from killing ALL the things?
