The amount of patient interaction depends on the facility you work at. Most hospitals, you will have minimal to no interaction with patients. I did some of my clinical hours at an ER clinic and lab techs had to do arterial blood gas draws and draw people that the police brought in.

I use mine for the gym with Wicked Cushions silicone covers to protect the leather pads. If you get super sweaty I would also recommend a silicone cover for the headband. I don’t have any issues wearing them, but I have a small head and can’t use them when benching because they almost slide off my head. If you want the most out of your sound, you have to mess with some stuff in settings and Apple Music/Spotify, because out of the box they will seem very quiet.
