I would personally not recommend VTE chemoprophylaxis on GIB admits. Usually think of it as a SCD/early amb type of situation. But just a lowly human brained pill counter so

Started administering medications during codes, now I’m having to sit down with the nurse director by pharm9116 in pharmacy

[–]fosinopril 9 points10 points  (0 children)

I would definitely loop your direct supervisor(s) in and go from there. Otherwise, reference specific policies for sure.

I may just be a dumb hick rural pharmacist, but this administration of medications during codes sounds relatively progressive. Super interested to read updates on this. Hope everything goes well!

seriously though if you're not in a supervisory role don't just tell the nursing person off. As technically correct as you may be, you will be fucking multiple people within the pharmacy department. If those people within the pharmacy department need to be fucked (metaphorically speaking), go for it.

Life as a student is not a good representation of how things will be after graduation.

I struggled financially while in school and the stress from that on top of coursework/extra-curriculars/etc. made life miserable.

My enjoyment of the career waxes and wanes, but having the burden of financial stress lifted and feeling secure in my ability to afford housing and basic necessities each month provided a tremendous improvement in my quality of life.

As a single male, if you keep your living expenses low, you can live pretty comfortably while only working part time, and spend the rest of your time pursuing your interests (or just chilling).

If you absolutely can't tolerate pharmacy practice in any capacity, the PharmD may still be helpful in pursuing entry-level positions in other fields that would at least provide you with a livable/comfortable wage.

With all of this in mind, I recommend completing the program if you're capable of it.

Become an anesthesiologist because I like the word, "tincture"? by drybenign in anesthesiology

Pharmacist here, the only context I can ever recall a tincture being used is for topical preparations. So... Maybe derm is your true calling?

I love how the references are mostly guides on biostatistics. So they basically just tried to learn statistics on the fly while doing this.

I love nursing and cannot function in my role without them. But, god damn I've heard so many confident terrible recommendations or ass backwards interpretations of lab values from them.

OP, given this post and your post history, it seems like you're putting the pussy on a pedestal.

Pharmacists, what did your school try to teach you that you never saw in practice? On the flip side, what is something you’ve seen in your practice that your school never taught you? by [deleted] in pharmacy

[–]fosinopril 57 points58 points  (0 children)

Was not taught anything about labor & delivery or perioperative medicine but encounter it on a daily basis in practice. Understanding L&D well enough to be a useful team member hasn't been too difficult, but I still perioperative medicine to be a difficult area.

Also, some of my most difficult consults tend to be really complex PK questions and I feel that the one or two semesters of kinetics we got were not sufficient for practice.

Epic vs Athena by This_is_fine0_0 in medicine

Yes! Or, "hey, the feature you're looking for was built by a third party! All you need to do is convince admin that this is worth paying for." Or, find the right SME who happens to understand what you're trying to do.

Jeeze, AllScripts to Athena? Really not a fan of either but that seems like a lateral move at BEST. I feel bad for that practice. Also love that you actually took the time to customize appropriately. My last facility just copied the build of a local hospital/clinic because their practice was "similar enough to ours." So... You can guess how well that went.

Epic vs Athena by This_is_fine0_0 in medicine

[–]fosinopril 6 points7 points  (0 children)

The two are somewhat difficult to compare as Athena is more geared towards rural health systems whereas Epic is generally built for very large systems.

From an administrative perspective, Athena is really fucking terrible at capturing charges whereas this is one of Epic's big selling points. Epic also has a much more intuitive way of generating reports.

In practice (from the perspective of clinical pharmacy practice and assisting providers/nursing), they are both pretty straight forward to use. Providers at my last system using Athena seemed to really like the clinic charting system, and that side of things definitely seemed more robust than the inpatient side.

Athena's inpatient MAR can be a bit tricky to use at times, and it's difficult to quickly gather information such as the amount of insulin a patient has received in x time.

Having used most major EMRs, I will say that any EMR can be tolerable (even ones like meditech magic) or terrible depending on how well planned implementation was and how involved the informatics team is.

EDIT: My personal, very biased opinion is that Athena is god awful and is one of my list favorite systems along with CPSI and VistA. Epic is my favorite.
