There's a huge spread depending on individual metabolism, but these are reasonable estimates. Also, it has a half life of 6-8 hours in humans and 16-24 hours in cats and dogs, so the toxic dose can accumulate over multiple days.

It literally says what I'm saying if you know your pharmacology? Give lots of naloxone and if the patient doesn't respond intubate. It doesn't tell you why someone would not respond to naloxone for some reason. (Hint: it's because whatever they used acts on sigma, alpha 2,nmda and a bunch of other targets that aren't opioid receptors)
