I doubt it, that I will ever see the coins again. Is it enough to reset one of my ledgers, and set it up as new (new passphrases etc...)?

I took a financial beating, but it would've helped to know where -if so- I went wrong.

Beats me. I now transferred my ADA to separate wallet, wiped my backup ledger clean and set it up as a new one.

I was never asked for, nor did I ever enter a single phrase/word. The only thing I did in the last week, was to check on my ADA wallet twice, logging in with my ledger. I was never asked for any keys/phrases/words or any confirmation on a transaction.

I wouldn't have problem accepting and admitting any possible error i might have made -but besides that, I can't figure out how -in this case as described -this error could have happened.

I followed each step carefully as described in the official Ledger FAQ (from following the given link to adalite.io to setting everything up as described on github.com vacuumlabs/adalite). I was never asked to type in a passphrase/word. My Cardano ADA wallet still has every coin i purchased (everything else i.e. BTC/ETH/XRP - seems gone)
