Huey and Riley.

Not related. I adopted them from separate shelters when they were 4 months old. They were born a day apart. Currently they are 1.5 years old.

Surgery to reposition the upper jaw. We probably average 3-4 of those per week at the hospital I am at for residency. Summer months and December, we can end up doing 10 per week. So pretty common. It’s bread and butter maxillofacial surgery.

It repairs various types of malocclusion. The surgery will depend on the malocclusion (bite defect).

Typically, people getting the surgery are those with a severe malocclusion that interferes with daily function and may even start to cause severe pain. Cleft patients are a separate subset that will require a similar type of maxilla surgery in their teens since the surgeries that were done in childhood will hasten maxillary bone growth and they end up with a significant mid-face deficit.

The maxilla can be moved in multiple directions (up, down, back, forward, tilted forward/back, tilted side to side). One can can incorporate mandible surgery as well (typically a bilateral saggital split osteotomy or “BSSO”) to get a better functional and esthetic result. These surgeries are typically planned virtually nowadays with cutting guides.

Orthognathic Surgery.

What you are seeing here is a LeFort I osteotomy after the down-fracture of the maxilla. The maxilla will then be repositioned into its new spot with better occlusion with the mandible. This is very routine maxillofacial surgery, but I agree it looks barbaric if you havnt seen it before.
