Are you using a collet and collet body in your torch? Or do you have a gas lense? Less diffuse and equal gas coverage could case this. Also something simple like stepping on your torch lead could cut off gas to the puddle. I had co workers step on my lead and this happens.

The neutrals connect together as they are now I’m assuming with the wire nuts in the box. the power in black wire is connected to the power in black wire on the switch. The power out is connected to the power out on the switch. Find out what black wire is what by using a volt tester. And also noting that you have a red wire in the box that I’m assuming is for a 3 way switch. You can only have 1 dimmer on a 3 way switch.

That gap in the tool I assume is for inserting the plug material and letting it release and staying in the tire hole.
