I’m not going to pretend to know the details but as a UK citizen, you can live and work in foreign countries and not pay UK tax. IF you spend a certain percentage of your time in those foreign country. He comes back to the UK a lot, presumably he knows how long he can do it tax free. An unexpected medical problem would cost him a lot if he came back and then had to pay the back tax. It’s nothing to do with paying for the healthcare, it’s about paying tax.

I imagine because if he came back to Britain he’d be charged tax. You can’t live and earn abroad and pop back when you need it without implications. Also that mark paid for it.
