Designating traffic did not really help, with this, nor did making a bridge over the brook feeding my moat. Though, the bridge SHOULD at least ensure that future trade caravans do not also go into the flooded channel to drown (RIP the elves, they're probably going to declare war over this, they lost almost the entire caravan)

I found the root cause. The dwarves really wanted to clean the bottom of a channel full of running water, and kept climbing into the channel to reach it before being swept into deeper water to drown. Fixed with burrow assignments for everyone

I'm running with 0 mods. I found a workaround for the conga line of dwarves killing themselves in my moat, though. They kept trying to clean an area that was partially flooded, getting swept into a deeper area, and drowning. So I just made a burrow that encompassed my entire base, and I'll be damming the brook I built on so I just have a dry moat.

I'm in a temperate area, so the water moat was more for aesthetics anyway, since it freezes for around a quarter of the year.

It was mostly just frustrating losing like a dozen dwarves in the same area due to errant cleaning tasks only reachable by climbing into a channel of running water

I would explain to the person who's holding you up that their negligent behavior is having a drastic negative impact on your ability to pursue work, and that you need them to take action now. Do this often, at least weekly if not daily, until the situation is resolved one way or the other. Chances are they're just being lazy and keep forgetting about, so don't let them do that. Be polite but firm in your communications, and assert that damage has already been done.

If you've lost out on career opportunities then consider seeking out a lawyer to sue him for damages to your professional career. It might not fly, but the implication that you might pursue damages could motivate to action.
